CENTRAL BOOKING 18.5 G.O. 18.5  TEQUESTA POLICE DEPARTMENT GENERAL ORDER TITLE: CENTRAL BOOKING GENERAL ORDER: 18.5 EFFECTIVE: March 1, 2003 RESCINDS: NEW PAGES: 3 CONTENTS: This order consists of the following numbered sections: I. ARRIVAL AT BOOKING FACILITY II. CENTRAL BOOKING PROCEDURE III.ARRESTS OF SCHOOL BOARD EMPLOYEES IV. ARRESTS OF PALM BEACH COUNTY STUDENTS PURPOSE: The purpose of this General Order is to establish guidelines governing the booking of prisoners into the Palm Beach County Jail. SCOPE: This procedure shall apply to all sworn Police Department personnel. PROCEDURE: I. ARRIVAL AT BOOKING FACILITY A. The arrestee shall be properly handcuffed by the transporting officer prior to entry into the jail facility. B. All arrestees shall be admitted through the Sallyport gate and must be accompanied by the transporting officer. C. Upon arrival, the transporting officer shall notify Communications via police radio of arrival at the jail. D. Upon entering the Sallyport area, the transporting officer shall secure his/her weapon in a security locker or the trunk of his police vehicle. The arrestee shall remain in the transport vehicle until removed by correctional staff. E. All restraining devices will be removed in accordance with the receiving facilities procedures. F. Upon transport or arrival at the receiving facility, the transport officer or receiving facility will be advised of any potential medical problems or security hazards. II. CENTRAL BOOKING PROCEDURE March 1, 2003 1 G.O. 18.5  A. Preparation of Sheriff’s Office Booking Form 1. The Sheriff’s Office Booking Form shall be completed prior to arrival at the booking facility. If this is impracticable, the Sheriff’s Office Booking Form shall be completed immediately upon arrival, having priority over other booking procedures. 2. All spaces on the Booking Form shall be completed with appropriate entries, not simply a line drawn through a space (i.e., non, not applicable, unknown.) 3. The Booking Form must be completed in its entirety to the satisfaction of the correctional staff. . B. The arresting officer shall contact Communications and obtain the following information: 1) Arrest Number; 2) Complaint Number; and 3) Times of Offense, Arrival, and Arrest. 1. This information may be obtained prior to arrival at the booking facility via police radio, telephone, etc. 2. When obtained, this information shall then be included on the Sheriff’s Office Booking Form. C. Completion of Probable Cause Affidavit 1. The Probable Cause Affidavit shall be completed in the following manner. 2. The arresting officer shall make sure that the following appears directly under the first formatted line of the PCA: TEQUESTA POLICE DEPT. 3. The arresting officer shall be responsible for the completeness of the Probable Cause Affidavit. All charges and statute numbers shall be verified for accuracy and the affidavit shall provide probably cause for the charges listed. 4. The Probable Cause Affidavit is used to explain probable cause to the judge. In order to substantiate probable cause, all of the elements of the crime must be met. These elements shall be addressed on the affidavit. 5. The arresting officer shall sign the affidavit and have it notarized by correctional staff. 6. The transport officer or the receiving deputy shall sign the PC Affidavit acknowledging the transfer of the , prisoner/detainee. Copies of the Rough Arrest, Probable Cause AffidavitUniform Traffic Citations (if applicable), Warrant Confirmation (if applicable), and Medical Clearance (if applicable), shall accompany the prisoner/detainee D. The arresting officer shall verify and sign the Palm Beach County Sheriff’s Office property receipt as completed by correctional staff. E. The Miranda Rights Form shall be completed by the arresting officer. F. In the event that criminal Uniform Traffic Citations are issued in the course of an arrest, a “Florida Uniform Traffic Citation Transmittal Form” shall be completed by the arresting officer. Only the charging criminal citation(s) shall be recorded on the transmittal form. Non criminal citations which provide probable cause for the stop and/or arrest are not to be listed on the transmittal form and will be handled in the following manner: 1. The arresting officer must document the issuance of the citation(s) on the Probable Cause Affidavit 2. Copies of the Probable cause citations are to be included in the arresting officer’s misdemeanor/felony case package. March 1, 2003 2 G.O. 18.5  A copy of the transmittal form shall then be submitted to correctional staff and the original shall be returned to the Records Section along with the blue NCR copies of all the criminal Uniform Traffic Citations. G. One (1) copy of all paperwork pertaining to the arrest shall be submitted (collectively) to the Records Section, including all arrest-related paperwork. H. Completion of DUI Probable Cause Affidavit: The same procedure will be followed as with other probable cause affidavits to include the following: 1. The officer shall issue a DUI citation. The citation number shall be noted on a transmittal form, along with any other criminal Uniform Traffic Citations. When a DUI citation is issued, “DUI” shall be noted in the remarks column of the transmittal form. 2. The following paperwork need to be turned into records immediately so that Records may in turn forward the necessary paperwork to the Driver Improvement Office in Fort Myers within 5 days: a. The GREEN portion of the DUI citation b. The BLUE portion of any other UTCs issued c. An original Implied Consent Warning . d. A copy of FDLE/ATP Form 23, Intoxilyzer Operational checklist (if test was conducted). e. A copy of FDLE/ATP Form 14, Chemical Breath Test Result Affidavit f. The Driver’s License of the defendant. If you cannot obtain the Driver’s License, an explanation must be included on the DUI citation. g. An original notarized Affidavit of Refusal Form (HSMV 72054) (if defendant refuses.) h. A notarized copy of the Probable Cause Affidavit. i. A copy of the Intoxilyzer printout (if applicable.) j. A copy of HSMV 72054, Chemical Test Report (if blood was drawn.) k. A copy of the Alcohol Influence Report. J. If blood has been drawn and an arrest is made at a later date, the arresting officer shall submit any of the above listed items to the Records Section, if applicable. The DUI Citation shall be completed and the officer shall list at the top portion of the citation the date of the original incident and shall enter the date of the arrest on the DUI citation where ARREST DELIVERED TO and DATE is listed. These two sections are approximately halfway down on the DUI Citation. K. Officers shall make note and advise the Records Section when supplies are low (i.e., fingerprint cards, transmittal forms, rights forms, etc.) III. ARRESTS OF SCHOOL BOARD EMPLOYEES A. It shall be the Records Section Supervisor’s responsibility to notify the Superintendent of Schools, within 48 hours, of the name and address of any employee of a school district who is charged with a felony, or with a misdemeanor involving the abuse of a minor child, or the sale or possession of a controlled substance. Notification to the Superintendent shall include the specific charge for which the employee was arrested (FSS 230.335). Officers shall make special notation to the Records Section Supervisor to satisfactorily accomplish this requirement. March 1, 2003 3 G.O. 18.5  IV ARRESTS OF PALM BEACH COUNTY STUDENTS A. It shall be the Records Section Supervisor’s responsibility to notify the Superintendent of schools, or his/her designee, of juveniles with the Palm Beach County educational jurisdiction arrested for crimes of violence or violations of law, which would be a felony if committed by an adult (FSS 985.04(7), 985.207(1)(b)). Officers shall make special notation to the Records Section Supervisor to satisfactorily accomplish this requirement. V.
