COMMITTEE The undersigned members of the subject Committee submit this report with sincere thanks for the opportunity to have participated in this very worthwhile project that will, if adopted, be a glowing addition to our Village that will serve forever as a tribute and benefit for all residents of our gxeat Village. We wish to express to you our combined, unqualified recommendation for your approval of the proposed Village of Tequesta Memorial as described herein. :kground Summary O 1 July 2002, the Village Council approved Resolution 62-01/02 that approved a Public Safety Memorial to be located at the new Public Safety Facility and established a Reserve Account to accumulate funds for its design and construction. It was understood that the Public Safety Memorial would be financed with funds donated for that purpose. To date approximately $18,400 have been accumulated from donations received for a dedication diruler and two golf tournaments. In January, 2004 Village Manager Couzzo proposed to then Mayor Resnik that the Village might be better served if the concept was. expanded to a Village of Tequesta Memorial. Then Mayor Resnik agreed to form and serve as Chairman of an Ad Hoc Committee of former Mayors Carlton Stoddard, Carl Hansen and Liz Schauer, with Assistant Village Manager Bob Garlo representing Village Administration, to review and evaluate the feasibility, design, estimated cost, operation and management of a Village of Teaquesta Memorial. The Committee would present its findings and recommendations to the Village Council for decision. ~I This plan was presented by then Mayor Resnik in a memorandum to the Village Council at the wary Council meeting. The Council provided unanimous consensus for the plan and t~~ ~ }ishment of the Ad Hoc Committee to accomplish the task and report their findings and recommendations to the Council. The Committee secured the participation of Mr Fred Menor, president of Lake Worth Monument t~ -provide guidance and assist in the design of the Memorial. Mr Menor has more than 25 years ~.. ~rience in the memorial and monument industry and is a third generation stone cutter with a well ea__.ed reputation for excellence. He has contributed significantly to the development of the proposed Village of Tequesta Memorial The Committee met in January and February and developed a set of proposed policies and procedures for the operation and management of a Village Memorial that are included in a later section of this report. The Committee also provided information to Mr. Menor to assist him in developing a proposed design for a Memorial. He has provided a beautiful design for a Village Memorial which the committee unanimously recommends (shown inTabs A and B to this report). At the April meeting of the Village Council, Councilmember Humpage stated that he considered the scope of the Village Memorial exceeded what was originally intended as a Public Safety Monument for which donated funds had been received. He requested that Council revisit the Village Memorial concept at the next Council Meeting. Prior to the May Village Council meeting, the Committee, met on 29 April to review funding for the Village Memorial.and agreed that the Village Memorial should be supported by public funds .The Committee acknowledged Councilmember Humpage's concern to be valid, and agreed that the ige Memorial and the Public Safety Monument should be separately designed, funded and cc ructed and that no funds donated for the Public Safety Monument be used for the Village Memorial. The Committee revised the format of the Village Memorial to eliminate Public Safety provisions. The estimated cost of the Village Memorial is anticipated to be less than $40,000. The foregoing Committee consideration and recommendation was presented in a report to the Village Council by Councilmember Resnik during the May 2004 Council meeting. Council provided unanimous concensus for separation of the two Memorials and continuation of the Conunittee's development of a proposed plan for a Village Memorial. The Committee held a meeting on 25 May to review this report and agree that the tasking given the Committee has been completed. The Committee unanimously agreed that the incorporation of a Village Memorial in the Tequesta Municipal Complex is not only feasible but a beautiful, highly desirable and beneficial addition to our Village that should be located to provide a quiet, peaceful place for individuals to sit in reverence and remembrance of their loved one whose name is inscribed on the Memorial. Village Manager Couzzo has indicated that he intends to incorporate the construction of the Village -norial as part of the overall Village Hall project so that the memorial area, monument base, w~ days and landscaping can be incorporated into the plan for the Village Hall municipal area. I'he Committee unanimously recommends that Lake Worth Monument Company be contracted to wild the Memorial Monument for the Village Memorial. Proposed Policies and Procedures for the Operation and Management of a Village Memorial Vames that may be inscribed on the Memorial >Deceased resident of Tequesta >Deceased loved ones of resident of Tequesta even if the deceased was not a resident >Deceased former elected Officials of Tequesta even if deceased was no longer a resident >Deceased former Village employee not a resident but that had 10 years tenure as employee >Deceased loved ones of Village employee not a resident but that has l0yrs tenure as a Village employee even if the deceased was not a resident B. Who may request a name be inscribed on the Memorial >A resident of Tequesta >Next of kin of a deceased resident, even if the next of kin is not a resident of Tequesta. >Village employee who does not reside in Tequesta but has 10 years tenure with the Village >Next of kin of deceased elected Official of Tequesta even if the next of kin is not a resident >Next of Kin of deceased Village employee that had 10 years tenure but was not a resident even if the next of Kin is not a resident C. Procedure for requesting a Memorial inscription >Applicants will use a standard form designed and provided by Village Administration >The form will specify what can be inscribed (name- dates) >The form will specify the cost of inscription >Completed form with payment (check) will be turned in to the designated Village Office D. Method of payment for Memorial inscriptions >Payment will be by check made out to Village of Tequesta E. Honorary inscriptions that do not require pa ment >Inscriptions for deceased elected Officials and deceased Village employees with 10 years tenure ~ F. Procedures for managing applications and payment for inscriptions >Village Manager will designate specific procedures for the following *Receiving and administering applications and payment checks *Contracting, supervising and paying for Memorial inscriptions *Maintenance of a Village Memorial Account G. Method of financing sustained operation and maintenance of the Memorial >Village Manager to include in the price of each inscription an amount to cover O & M. >Include in the annual Public Works Budget funds to provide for Memorial maintenance >Include in the annual Public Works Budget funds for landscape maintenance 3 Description of the proposed Village Memorial Monument, Memorial Platform and Loge ThP Committee met with Mr Menor to review the purpose of the Memorial, discuss basic design cr _ ~a. and tour the Village Green to offer Mr Menor an idea of the general location of the Me....,rial. With this background, Mr Menor designed a beautiful Memorial with a main monument crested with an impressive bronze bust of a Tequesta Indian.(Tabs A and B) . A. Description of the proposed Memorial Monument (see Tab A~ >The center main monument is 6' 10" tall and 6' long at the base. Along with the bust of the Tequesta Indian, title & dedication phrase, this monument can accommodate 128 names >A one half inch deep recess near the bottom of the tablet and a sub base help to give the the main monument height and character. >The wings of the monument are for inscriptions only. Each wing is 8' 6" long and 4'2" high. The top of each wing is beveled to permit more inscriptions while keeping the height of the wings well below the central monument. This difference in height also prevents a feeling of being "walled in" while viewing the inscriptions individually. >The wings (or tablets) are set on three smaller sub-bases which are set on the main base Which is 8'10" long. An area of open space is left between the wings and the central monument to create a clean, contemporary, attractive look and provide for landscaping. > The unpolished panel areas (white areas on the rendering) allow for clean, readable inscriptions, however small they may be. > Each wing has the potential of accommodating 360 names. The total number of names for the two wings and the central monument is around 848. The addition of three more wings in the future would allow for 1080 more names for a memorial total of 1,928 names >The entire memorial monument will be constructed of gray granite. All surfaces except the base and the inscription panel areas are to be polished. The inscription areas will be left unpolished with polished granite borders. Inscriptions will be made in black on the unpolished granite ~• Description of the proposed Memorial Platform (see Tab A and Tab B~ >The memorial platform would be constructed of paver brick similar to that being used for the walkway between the Public Safety Building and the Village Hall. The platform would be circular, 20' in diameter, accessed by one or two paver brick walkways. Landscaping within the platform area and extending to a total diameter of 35' to 40' is recommended to complement and enhance the overall beauty of the memorial area. >In the center of the memorial platform there would be a small circular open area landscaped with small plantings. >Two semicircular benches made of solid concrete 18" to 20" high would be constructed around the circular open area as shown on Tab B. It is planned that these two benches will be topped with marble saved from the original Village Hall. ~~'he circumference of the 20' memorial platform would require poured concrete bases upon -which the main monument and monument wings would be placed 4 Description of the proposed bronze bust of the Tequesta Indian logo (Tab Aj >The Tequesta Indian bust will. be cast in bronze in a 2 %i" to 3" bas relief. >The bust will have no wording and will be approximately 18" to 20" tall > The bronze bust will be recessed in the rounded top of the main memorial monument ~ The shape of the top of the main memorial monument will highlight the bronze bust > A clay rendering of the bust will be provided for approval prior to casting > The artisan proposed for casting the bust is an expert and reasonable in price > A separate contract to produce the bronze bust is needed this year to obtain this artisan ~ Additional relevant information > Mr Menor offers to provide the Village ownership of detailed design drawings and all specifications for memorial monument, wings and base platform for a total fee of $900. > Mr Menor would deduct this design fee from the price of the memorial monument if Lake Worth Monument is contracted for this project. > Mr Menor will cut the inscriptions of individual names on a periodic request basis in accordance with specific details coordinated and agreed with Village Administration. He will require a minimum order for six inscriptions per trip. > Mistakes cannot be patched or erased, so the Village will be provided with proofs prior to final sign off for the inscription(s). > The Village charge for an individual Inscription should be less than $40 > The main monument will have room for special inscriptions : a title line i.e. "Tequesta Memorial": The date of dedication of the memorial: a dedication phrase i. e. "Dedicated With Loving Memory " and a scripture quote from the book of Psalms i. e. the " 23`d Psalm". are recommended by the Committee for Council consideration 'onclusions As previously stated in this report, This Committee unanimously and enthusiastically Supports the inclusion of a Village Memorial, as described herein, to be constructed on the village Green as an integral element of the future Village Municipal Complex. The Village should immediately purchase the design drawings of the memorial and request a cost proposal for the memorial monument and bronze bust of the Tequesta Indian from Mr. Menor. and include construction of the memorial platform in the Village Hall Project. Provide colorful photos of the finalized renderings of the Tequesta Memorial, with description of its historic value and beauty for our entire community -- together with photos of the new Village Hail -- to be printed prominently in the next issue of Smoke Signals --plus feature stories by newspapers and TV -- inviting them io interview village managers Cuozzo and Carlo for further salient facts -- with authorization of high lif# aerial photos of the new Village Hall and beautifully landscaped Village Green where the Tequesta lyiemorial will be located. ~', restful place to sit with sluiet reverence and prayer for Loved ones engraved in our memory. 2. Invite Tequesta families to request that names of their beloved deceased family members who were Tequesta residents, be engraved on granite at the Tequesta Memorial, in loving remembrance. Moderate cost of engraving the inscriptions to be paid by the family, with added cost for extra lines more than two. A letter to all Tequesta families, with attractive photos of the Tequesta Memorial, would e_r~sure that ell Tequesta families receive the invitation for inscription of names. 3. The suggestion by Mayor Schauer has merit, that words from the 23rd Psalm be inscribed beneath the Tequesta Memorial headline title. Words from the 4th paragraph would fit nicely -- ~e~, thvugh I walk through the valley of the shadvw of death, I w~~l fear nfl evil, for thou art with me ... 4. Recognizing the historic importance of Mayor Resnik's achievement for Tequesta as chairran of the Tequesta Memorial committee, I would like to respectfully propose that his name be the first inscribed on the Memorial. The inscription would be on the first left-side panel beneath the indianhead sculpture. I would like to propose that this recognition of Mayor Resnik's leadership in csaeating the Tequesta Memorial be approved by vote of the committee. Mrs. Edward Resnik (Alicia) should be consulted on wording of the inscription , which I suggest be four lines at no cost for the Resnik family. One possible wording might be -- CoL Edward Resnik Tequesta Mayor 2405 - 2006 (check the dates) Interment with honors at Arlington National Cemetery 5. I would also Tike to respectfully submit a motion to appoint Tequesta Assistant Manager Robert ~s~rlo as chairman of the Tequesta Memorial Committee, succeeding Mayor Resnik. Mr. GarIo would be close to Village Manag®r Michael Cuozzo in finalizing and constructing the Tequesta Memorial on the north side of Village Green park . 6. The Stoddard family, represented by our son Allan M. Stoddard, our Family Trustee, would like to be among the first to pay for inscriptions of family names, to be on the Memorial center panels. Allan delivered a check for $100 to village offices a year ago, fora 2-line inscription for his mother, my beloved wife, to read -- Alberta Hoppe Stoddard 1910 - 2002 Allan 2s submitting herewith a second check for $ l OD fora 2-line inscription to read -- Carlton Dunlap Stoddard . Tequesta Mayor 1983 - 1988 Please leave a 2-line space next to Alberta's, to be inscribed after my death. Any over-payment need not be returned. Please retain it in the Tequesta Memorial fund. Like Mayor Resnik, our son Allan will also be interred at Arlington National Cemetery, honored for his leadership in the computer system for 1VATt3, at Frankfort, Germany, and after retirement from 2Q years in the U.S. army, as Senior Engineer for TRW in national defense. RESOLUTION NO. 62-01/02 A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF TEQUESTA, PALM BEACH COUNTY, FLORIDA, ESTABLISHING A PUBLIC SAFETY MEMORIAL RESERVE, FOR THE PURPOSE OF ACCUMULATING FUNDS FOR THE DESIGN AND CONSTRUCTION OF A POLICE AND FIRE MEMORIAL TO BE LOCATED AT THE NEW PUBLIC SAFETY FACILITY. NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF THE VILLAGE OF TEQUESTA, PALM BEACH COUNTY, FLORIDA, AS FOLLOWS: Section 1: The Finance Department is hereby directed to establish a reserve account, the Public Safety Memorial Reserve. Section 2: The Public Safety Memorial Reserve will accumulate funds to finance the design and construction of a Police and Fire Memorial to be located at the new Public Safety Facility. The foregoing Resolution was offered by Councilmember von Frank, who moved its adoption.
