Dear Mr. Cuozzo:
Enclosed, as requested and originally submitted on December 12,2017, is a Work Authorization to cover
construction services for the above-referenced project. Actually, the original work authorization included
the scope for engineering construction services but anticipatetl fee approval at the end of the design
period. It is noted that bidding services are now complete so those services are not included in this
authorization. In addition to the authorization, I have also enclosed a breakdown showing how the
proposed estimate was developed.
If acceptable, please execute and return one copy to our office, In the interim,should you have questions
or require anything further, please call.

UTILITY ENGINEERING SERVICES WORK AUTHORIZATION
WTP MtSCEL�ANEOUS UPGRADES
4n December 2, 2016, Kimiey-Nam and Assaciates Inc. {Cansultant} received a
purchase arder from the Village of Tequesta (Ciient} to perform Phase I design senrices
far the "1NTP — Miscellaneaus Upgrades" project. A capy of the authorization and
purchase order is attached. This autharization established a scope for Phase II services
which includes Constructian. The intent was to authorize Phase li services at the
conclusian of Phase 1. The first phase and the bidding phases are now complete. An
award ta initiate the construction phase is naw imminent.
The purpose of this is to authorize Phase II services (exclusive of bidding services) as set
forth in the ariginai authorizatian. The services cavered by this authoriza#ion are subject
to aH terms and conditions as set forth directly, or by reference, in the Phase I
authorization. The following descriptions provide additianal detail to the services to be
prouided under ltems 3, 4, 11, and 24 as contained in the Phase II sectian of the original
authorization.
Visits to Site and C3bservation of Construction. Consultant will make visi#s in order to
observe the progress af the Work. Such visits and observations by Consultant are nat
intended to be exhaustiue or#o e�end to every aspect of Contractor's work. (7bservations
are to be limited to spot checking, selective measurement, and similar methods of general
� observation. Based on information obtained during such visits, Consultant wiil evaluate
whether Contractor's work is generaHy proceeding in accordance with the Contract
Dacuments, and Gonsultant will Keep Glient infarmed of the general progress of the Wark.
Consultant shall not, during such visits or as a result of such observations, supervise,
direct, or have control over Contractor's work, nor shai( Consultant have authority over or
responsibility far the means, methads, techniques, equipment choice and usage,
sequences, schedules, or procedures af canstruction selected by Contractor, for safety
precautions and programs incident to Contractor's work, nor for any failure af Contractor
to comply with applicable laws and regulations. Consultant neither guarantees the
perfarmance of any Cantractor nor assumes responsibiiity far any Contractor's failure to
fumish and pe�farm its work in accordance with the Gontract Documents.
Applications for Payment. Based on its abserva#ions and on review of applications far
payment and accampanying supporting documentation, Cansultant wilf determine the
amaunts that Gansultant recommends Cantractor be paid. Such recommendations wiil
be based on Consultant's knawledge, information and belief, and will state whether in
Consultant's opinion Contractor's work has progressed to the point indicated, subject to
any quafifications stated in the recammenda#ion. Consultant's recommendafions wiA not
be a representation that its observatians ta check Contractar's work have been
exhaustive, e�ended ta every aspect of Contractor's work, ar involved detailed
inspections.
Fina1 Notice of Acceptability of fhe Work. Cansultant will conduct a final site visit ta
determine if the completed Work of Contractor is generally in accordance with the
Contract Docurnents and the finai punch list so that Cansultant may recommend, in
writing, final payment to Cantractor. Accompanying the recommendation for final
payment, Consultant shall also provide a na#ice that the Wark is generaliy in accordance
with the Contract Documents to the best of Gonsultant's knowledge, information, and
belief based on #he extent of its services and based upan informatian pravided ta
Cansultant upon which it is entitled to rely.
As described, the Constructian Phase services will be perfarmed over an assumed 11
month construction period an a lump sum basis for$122,9 50.OQ.
