Approve Jupiter Medical Center Urgent Care 2020-2022 Agreement For Services
The Village continues to require a healthcare agreement with a competent facility to provide
occupational health services inclusive of applicant screenings, post -accident drug testing and
physicals, and to also help manage our workers compensation injuries from initial treatment through
return to work status.
An updated Agreement for Services with amended service fees are attached as Exhibit A. There are
no changes to the main services of general physicals and drug testing, however of the twenty
services currently offered, two experienced a reduction in cost, while eleven were increased by a
range from $2 to $30.
The continued use of JMC's Urgent Care services is important for the additional reasons outlined
below:
1. JMC Urgent Care is the closest occupational health facility offering the most comprehensive
set of services within a 10-mile radius (6.4 miles away) due to their size and available
resources.
2. The regular opening hours are wide-ranging from Monday to Saturdays, 8.00am- 8.00p.m and
Sundays, 9am-5 p.m., with JMC Emergency room as an alternative resource.
3. Continuity of service with a provider is important in respect of available medical history and
long-term employee treatment, especially as it relates to WC handling.

renewed for 2 years for the period October 1, 2020 to September 30, 2022.

AGREEMENT FOR SERVICES
This Agreement for Services (this "Agreement") by and between Village of Tequesta
("Company") and Jupiter Medical Center Physicians Group, Inc. ("JMC Urgent Care") effective as
of the date the last of which both parties has signed below 1010112020, and describes the parties'
agreement with respect to the following: expiration on 0913012022.
SCOPE OF SERVICES
Scope of Service. JMC Urgent Care will provide Occupational Health medical services (the
"Services") to those of Company's employees who have been approved to receive Services
under this Agreement by presenting to JMC Urgent Care with a completed Company referral
form ("Company's Recipients"). The Services which may be provided to Company's Recipients
are set forth in the Services Fee Schedule attached to this Agreement.
Additional Services. JMC Urgent Care will provide additional services upon request on terms
mutually agreed to by the parties. The Services Fee Schedule may be amended from time to
time to include any services added to this Agreement.
Location. Services may be performed at any JMC Urgent Care location, unless mutually agreed
upon otherwise and/or if Company requests that Service accessibility by Company's Recipients
be limited to specific locations.
GENERAL TERMS
Term of Agreement. This Agreement shall begin on the Effective Date and shall automatically
renew on October 1st each year, unless Company delivers written notice of termination to JMC
Urgent Care prior to the start of a renewal year. Company acknowledges that JMC Urgent Care
shall have the right to update pricing in the Services Fee Schedule not more than once each
renewal year and shall notify Company of such changes within thirty (30) days of such change.
Cancellation. This Agreement may be canceled by either party at any time by giving the other
party at least thirty (30) days' advance written notice of such termination.
Fee for Service. Company agrees to pay fees to JMC Urgent Care for all Services rendered to
Company's Recipients in accordance with the Services Fee Schedule.
Authorization Forms. Referral forms from the Company will be required before any Services
may be rendered to Company's Recipients under this Agreement. Where required by state or

federal law, authorization of the Company Recipient receiving the Services will be required
before such Services are rendered.
FINANCIAL RESPONSIBILITY
Billing and Payment.
Invoices shall be submitted monthly to Company and shall identify the Company Recipient,
date(s), Service(s) rendered, and corresponding fees in the Services Fee Schedule. Company
shall remit all payments due to JMC Urgent Care within thirty days (30) of receipt of an invoice.
Late Payments.
A payment is considered late 90 days after the Service date and is subject to a 1.5% per month
late fee.
PUBLIC RECORDS. In accordance with Sec. 119.0701, Florida Statutes, CONTRACTOR must keep and
maintain this Agreement and any other records associated therewith and that are associated with
the performance of the work described in the Proposal or Bid. Upon request from the Village's
custodian of public records, CONTRACTOR must provide the Village with copies of requested
records, or allow such records to be inspected or copied, within a reasonable time in accordance
with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR who fails to
provide the public records to the Village, or fails to make them available for inspection or copying,
within a reasonable time may be subject to attorney's fees and costs pursuant to Sec. 119.0701,
Florida Statutes, and other penalties under Sec. 119.10, Florida Statutes. Further, CONTRACTOR
shall ensure that any exempt or confidential records associated with this Agreement or associated
with the performance of the work described in the Proposal or Bid are not disclosed except as
authorized by law for the duration of the Agreement term, and following completion of the
Agreement if the CONTRACTOR does not transfer the records to the Village. Finally, upon
completion of the Agreement, CONTRACTOR shall transfer, at no cost to the Village, all public
records in possession of the CONTRACTOR, or keep and maintain public records required by the
Village. If the CONTRACTOR transfers all public records to the Village upon completion of the
Agreement, the CONTRACTOR shall destroy any duplicate public records that are exempt or
confidential and exempt from public records disclosure requirements. If the CONTRACTOR keeps
and maintains public records upon completion of the Agreement, the CONTRACTOR shall meet all
applicable requirements for retaining public records. Records that are stored electronically must be
provided to the VILLAGE, upon request from the Village's custodian of public records, in a format
that is compatible with the Village's information technology systems.

Pursuant to Article XII of the Palm Beach County Charter, the Office of the Inspector General has
jurisdiction to investigate municipal matters, review and audit municipal contracts and other
transactions, and make reports and recommendations to municipal governing bodies based on such
audits, reviews, or investigations. All parties doing business with the Village shall fully cooperate
with the inspector general in the exercise of the inspector general's functions, authority, and power.
The inspector general has the power to take sworn statements, require the production of records,
and to audit, monitor, investigate and inspect the activities of the Village, as well as contractors and
lobbyists of the Village in order to detect, deter, prevent, and eradicate fraud, waste,
mismanagement, misconduct, and abuses.

The "Quick Fix":
Cost: $390

Home/Office Computer Workstation
Will provide brief ergonomic report listing the existing risk factors and recommendations,
including workplace re -design, low and no -cost solutions and personal ergonomics.
Follow -Up to Quick Fix:
Cost: $163
A follow-up visit can be performed to ensure proper adjustment of new equipment
and reinforcement of proper body mechanics and stretching instructions. Travel costs per
mile over 10 miles radius from Jupiter Medical Center.
Ergonomic Training program for office employees:
Cost: $585
On site 1-hour hands on learning program on Ergonomics and injury prevention for back,
neck, and arm. Maximum number of participants: 10. Call for pricing of more than 10
participants." Training materials included.
Worksite Ergonomic Job Analysis:
Cost: Starting at $1,235 per job area
Walkthrough checklist, on site collection of data, offsite analysis of the gathered materials
and generation of the written Ergonomic Report including. Abatement strategies and
recommendations of ergonomic solutions will be provided. NIOSH lifting equation, AND Z-
365, NSC Risk factors checklists and other assessment forms will be used when deemed
appropriate.

Will provide an ergonomic report listing an overview of existing risk factors and
recommendations including workplace re -design, low and no -cost solutions and
personal ergonomics. If assessment of more than one workstation is required
and/or use of in-depth assessment forms (NIOSH lifting equation, ANZI-365, etc.)
are required then a "Worksite Ergonomic Job Analysis" may be billed for each
workstation/task.
