The existing vinyl flooring is worn and scuffed. The Vendor, Capitol Carpet & Tile and Window Fashions, is requiring a signature on their proposal. The total cost of the project for labor and materials is $1,072.08. Funding for this project is available in Fiscal Year 2021 in the Building Repair and Maintenance account 001-210-546.303. PUBLIC RECORDS. In accordance with Sec. 119.0701, Florida Statutes, CONTRACTOR must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Proposal or Bid. Upon request from the Village's custodian of public records, CONTRACTOR must provide the Village with copies of requested records, or allow such records to be inspected or copied, within a reasonable time in accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR who fails to provide the public records to the Village, or fails to make them available for inspection or copying, within a reasonable time may be subject to attorney's fees and costs pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Proposal or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and following completion of the Agreement if the CONTRACTOR does not transfer the records to the Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to the Village, all public records in possession of the CONTRACTOR, or keep and maintain public records required by the Village. If the CONTRACTOR transfers all public records to the Village upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records that are exempt or confidential and exempt from public records disclosure requirements. If the CONTRACTOR keeps and maintains public records upon completion of the Agreement, the CONTRACTOR shall meet all applicable requirements for retaining public records. Chambers, Director Public Works Subject: Irrigation Backflow Replacement Date: June 21, 2021 561-768-0700 www.tequesta.org The following agenda item references installing a new 2" Irrigation Backflow device on the irrigation system on Seabrook Road. The Village of Tequesta irrigates the grass in the Village right of way on both sides of Seabrook =H The existing 2" backflow device is defective and requires replacement. A backflow device is required to protect potable water supplies from contamination due to backflow. The backflow preventer is designed to prevent the water in our main water supply lines from flowing in a reverse direction. Jupiter Plumbing Services is locally licensed plumbing contractor and certified to test, repair and replace backflow devices. The spigot is leaking from a worn o-ring and stem. Jupiter Plumbing will rebuild the wall hydrant with a new stem and o-ring kit. The total cost for labor and materials is $426.28. The old drinking water fountains were defective, not ADA complaint and removed from service. A new ADA Water Fountain will be installed at the Park bathrooms. The service laterals feeding the old water fountains will be removed and capped at the water main. Water cannot remain stagnant in a dead-end pipe. Water that travels through pipe to a dead end is not considered to be best practice and should be avoided. Dead end pipe reduces fire flows and leads to the deterioration of water quality and can cause corrosion. Multiple leaks were identified in the Public Safety Facility. Roof Tiles will be removed down to the plywood decking to inspect for rotted wood. Rotted wood will be replaced, the membrane and tiles replaced. Wolverine Roofing is locally licensed roofing contractor extending competitive pricing for labor, materials and repairs. The total price for repairs is $4,517.52. Funding for this project is available in Fiscal Year 2021 in the Building Repair and Maintenance Account, 001-210.546.303. PUBLIC RECORDS. In accordance with Sec. 119.0701, Florida Statutes, CONTRACTOR must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Proposal or Bid. Upon request from the Village's custodian of public records, CONTRACTOR must provide the Village with copies of requested records, or allow such records to be inspected or copied, within a reasonable time in accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR who fails to provide the public records to the Village, or fails to make them available for inspection or copying, within a reasonable time may be subject to attorney's fees and costs pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Proposal or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and following completion of the Agreement if the CONTRACTOR does not transfer the records to the Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to the Village, all public records in possession of the CONTRACTOR, or keep and maintain public records required by the Village. If the CONTRACTOR transfers all public records to the Village upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records that are exempt or confidential and exempt from public records disclosure requirements. If the CONTRACTOR keeps and maintains public records upon completion of the Agreement, the CONTRACTOR shall meet all applicable requirements for retaining public records. Records that are stored electronically must be provided to the VILLAGE, upon request from the Village's custodian of public records, in a format that is compatible with the Village's information technology systems.
