Please find a partially executed copy of Work Authorization (WA) CM-STW21.01 with Chen -
Moore & Associates for your signature. This WA is for permitting services required for
stormwater improvements on River Drive and Gulfstream Drive in the amount of $20,540.00. The
FY20-21 Stormwater Enterprise Fund contains sufficient funding to complete this work
authorization.

PROJECT TITLE: Bioswales at Gulfstream and Riverside
This Work Authorization, when executed, shall be incorporated in and shall become an integral
part of the Agreement for Professional Services dated September 10, 2020 between the Village of
Tequesta ("VILLAGE"), and Chen -Moore & Associates ("CONSULTANT") hereafter referred to
as the Agreement.
I. PROJECT DESCRIPTION
CMA shall provide engineering assistance with the design of drainage improvements at the
following two locations:
1). Eastern swale along the northernmost extension of Gulfstream Drive, and
2). Southwest comer of Riverside Drive and Tequesta Drive
II. SCOPE OF SERVICES
The following is a description of the services to be provided under this Task Order.
TASK 1— Data Collection
CMA will review available as -built records, field photographs, perform a site visit, and
review other available records to determine the best approach to re -grade the existing Swale
and install the underdrain system. CMA will submit a Sunshine One Call design ticket to
determine the presence of underground utilities that may conflict with the proposed
improvements. CMA's sub -consultant (LIDBERG) will perform a limited property and
topographic survey of the impacted area. CMA will provide a short technical memorandum
documenting the findings of the data collection. An online meeting will be held to review
the findings and discuss issues or concerns.
TASK 2 — Draft Construction Exhibit
Based on the collected data, a draft construction exhibit will be prepared by CMA to be

used by Village field staff or pre -approved vendors to construct. It is assumed that the
construction exhibit will consist of the following sheets:
• Cover Sheet
• Plan Sheets (1 for Each Site)
• Detail Sheet with Typical Section
A draft of the construction exhibit will be provided for review and comment by Village
staff. An online meeting will be held to review the plan and discuss issues or concerns.
TASK 3 — Permitting
For the Gulfstream Drive location an Environmental Resources Permit (ERP) application
will be prepared based on the proposed improvements and under the assumption that a
review of the entire system is not necessary. For the Riverside Drive location, a letter
modification of existing permit 50-00682-S will be filed. It is assumed that one (1) Request
for Additional Information (RAI) letter will be responded to in the process of procuring the
permits. This task does not include preparation of a general ERP for any portion of the
stormwater management system beyond the limits of the proposed bioswale improvements.
TASK 4 — Final Construction Exhibit
Based on the comments of Village staff on the draft submittal and the comments and
conditions of the approved permit, CMA prepare a final submittal for use by Village field
staff or pre -approved vendors to construct. It is assumed CMA will not be required to
prepare documents in a manner to support advertisement for bid of the work.

The anticipated schedule is based on issuance of the Notice to Proceed and is projected as
follows:

IV. EXCLUSIONS
The services outlined below are not included as part of the scope of work, although
additional service agreements can be executed to assist the Village with these services if
necessary:
1. Design of any improvements south of the eastern driveway at 374 West Riverside
Drive, west of the existing curb inlet at the southwest corner with Tequesta Drive,
east of Riverside Drive or north of Tequesta Drive.
2. Design of any improvements north of the driveway for 148 Gulfstream Drive, south
of 140 Gulfstream Drive, or west of Gulfstream Drive.
3. Detailed field work for underground utility locates.
4. Designing the replacement or modification of any existing underground utilities.
5. Permitting of any existing facilities beyond the limits of the proposed improvements.
6. Engineering services during construction or construction administration.

COMPENSATION AND PAYMENT
The CONSULTANT will perform the tasks as outlined in the scope of services for a Lump
Sum Fee of $20,540.00 as outlined in Table A.

AGREEMENT REFERENCE
The work authorized under this Work Authorization shall be performed under the terms
and conditions described within the Professional Services Agreement dated September 10,
2020, by and between the Village of Tequesta ("VILLAGE") and Chen -Moore &
Associates ("CONSULTANT"). Compensation shall not exceed the hourly rates currently
in effect under this Agreement.
