During the course of construction several unforeseen conditions were discovered related to the
water main replacement portion of the project. As a result, additional professional services were
required of Mock Roos, the engineering consultant for the project. Please find a partially
executed copy of Amendment No. 1 in the amount of $6,500 for Work Authorization (WA) MR
UTL20.02 with Mock -Roos for your signature. The FY20-21 Water Enterprise Fund contains
sufficient funding to accommodate this amendment.

Proposal to Provide Professional Engineering Services for
Pinetree Drive Drainage Improvements and
Tequesta Drive Bridge Supplemental Revetment

Proposal Terms
A. Project Description:
The Village of Tequesta (Village) has identified the following two stormwater components that are to be
incorporated into one project and completed in Fiscal Year 2020:
Pinetree Drive Drainage Improvements
Mock -Roos was selected by the Village to design improvements to the Pinetree Drive drainage to reduce
localized ponding that occurs following rain events. The design completed in 2017 generally included
milling and resurfacing (grade adjustments), curb, inlets, pipe, and an outfall connection to reduce the
localized ponding. The design will be updated, the project will be publicly advertised for bid, and then
constructed. Minimum drawing updates are to include a proposed Swale on the north side of eastern most
sidewalk and proposed cross walk striping extending across 'Tequesta Drive.
Tequesta Drive Bridge Supplemental Revetment
In 2017, Mock -Roos was working with the Village on a conceptual plan to stabilize the stormwater
erosion that was occurring at the east side of Tequesta Bridge (north and south abutments). A conceptual
plan was developed to reinforce the eroding slope of the banks in these areas and allow a place for
ponding stormwater to flow with reduced erosion. The conceptual plan was completed in 2017 and the
design will be finalized, the project will be publicly advertised for bid, and then constructed.
Additional Services — Amendment No. 1
The Village identified an existing 8" asbestos cement watermain that was located within the proposed
improvements of the Pinetree Drive Drainage Improvement project. At the Village's request, the design
for the replacement of the existing asbestos cement watermain was incorporated into the construction
drawings and the proposed replacement watermain was permitted with the Palm Beach County Health
Department. Additional engineering during construction services were also provided to assist the Village
with shop drawings review, field observations, and the Palm Reach County Health Department permit

At the Village's request, Mock -Roos performed the following additional design, permitting, and contract
administration services as outlined in B. Scope of Services below:
B. Scope of Services:
Task 1— Additional Design and Permitting Phase Services
Design and update the construction drawings for the replacement of the existing 8" asbestos
cement watermain. Provide the Village with a watermain replacement drawing to be used for
bidding the project.
Submit a permit to the Palm Beach County Health Department for the proposed watermain
replacement.
Task 2 — Additional Contract Administration
1, Review Contractor's watermain replacement fabrication shop drawings and engineering
submittals for conformance with the Contract Documents.
2. Review and respond to a Contractor's Request for Information (RFI) associated with the
watermain and the extension of the watermain beyond the proposed limits of the pavement.
3. Assist the Contractor and Village with compiling, reviewing, and providing permitting closeout
documents to the Palm Beach County Health Department.
C. Additional Services:
1. Any services not included in the Scope of Services will be considered Additional Services.
2. Any design changes, schedule changes, drawing changes, or other project changes requested by
Client will be considered Additional Services.
3. Additional Services can be provided upon Mock -Roos receiving signed authorization from
Client.
4. Additional Services resulting from the following can be provided without Client's authorization:
t) legal and/or regulatory requirements or changes, 2) delays or price increase not in Mock -Roos'
control, 3) emergencies or acts of God, 4) threatened or endangered species, 5) hazardous
conditions, 6) or any services required to protect the health, safety, and/or welfare of the public.
D. Fees and Rates:
Mock -Roos will complete these services on an hourly basis at Mock -Roos' hourly rates, plus
reimbursable expenses.
2. The total fee to provide the Scope of Services is anticipated to be $6,500.

3, Mock- Roos can provide Additional Services at the Mock -Roos rates in effect at that time, plus any
reimbursable expenses, or for an agreed upon Iump suns fee.
E. Conditions:
1, All terms become valid upon Mock -Roos receiving one complete copy of this proposal within 60
days of its (late.
2. Phis proposal serves as a supplement to the general agreement between Mock -Roos and Client
dated May 12, 2016,
3. fn case of discrepancies, the tenns ofthis proposal supersede those of previous agreements.
F. Acceptance and Authorization to Proceed:
This proposal is acceptable and Mock -Roos has authorization to proceed with the Scope of
Services. This authorization becomes valid Upon Mock -Roos receiving one complete copy of this
form with an original signature below, .,
