The work will include the removal and disposal of the existing carpet and the preparation and installation of the new carpet. The Carpet will be replaced in four offices, totaling 53sy. The total cost for services is $2,877.70. Funding for these services is available in Public Works Account, 001-210-546.303, Building Repair and Maintenance. PUBLIC RECORDS. In accordance with Sec. 119.0701, Florida Statutes, CONTRACTOR must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Proposal or Bid. Upon request from the Village's custodian of public records, CONTRACTOR must provide the Village with copies of requested records, or allow such records to be inspected or copied, within a reasonable time in accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR who fails to provide the public records to the Village, or fails to make them available for inspection or copying, within a reasonable time may be subject to attorney's fees and costs pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Proposal or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and following completion of the Agreement if the CONTRACTOR does not transfer the records to the Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to the Village, all public records in possession of the CONTRACTOR, or keep and maintain public records required by the Village. If the CONTRACTOR transfers all public records to the Village upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records that are exempt or confidential and exempt from public records disclosure requirements. If the CONTRACTOR keeps and maintains public records upon completion of the Agreement, the CONTRACTOR shall meet all applicable requirements for retaining public records. Records that are stored electronically must be provided to the VILLAGE, upon request from the Village's custodian of public records, in a format that is compatible with the Village's information technology systems. Village 345 Tequesta Drive Tequesta, FL 33469 of Tequesta To: Jeremy Allen —Village Manager Through: Matthew Hammond — Utilities Director From: David Tinoco — Distribution Superintendent Date: August 20, 2021 Re: Bioswale Installation — Recommendation of Award 561-768-0700 www.tequesta.org Beginning in Fiscal Year 20 the Utility Department has been retrofitting existing swales within Village right-of-ways with bioswales to improve drainage and nutrient removal. The areas selected for this work has initially been locations that experience localized ponding during high rain events. The next phase of areas identified for bioswale retrofit are on Beacon Street (between Cedar Ave and Seabrook Rd), Riverside Drive (near Tequesta Dr) and on Fairview East. To complete this work quotes were requested from qualified contractors to install approximately 347 linear feet of bioswale at these locations. Note that all quotes include course gravel, engineered biosoil, drainage pipe and filter fabric to be provided by the Village. Three vendors responded (Inland Grading, Sunshine Land Design and Shenandoah General Construction) with Eddie Huggins Land Grading declining due to work load. Each quote was reviewed by Utilities staff for adequacy and the tabulation of quotes is below: VENDOR SCOPE QUOTES NOTES Inland Grading, Inc. Inland Grading successfully completed the previous phase of bioswale retrofits earlier this year. The approved Fiscal Year 21 Stormwater Enterprise Fund budget includes $56,000 for swale restoration projects, $30,000 of which coming from a Florida Department of Environmental Protection Non -Point Source grant. This WA, in the amount of $24,925.00, is for consulting engineering services to develop a stormwater management guidance manual. This manual will assist new development within the Village to apply low -impact development stormwater principles. Chambers, Director Public Works Subject: Tequesta Drive Bridge Underwater Inspection Services Date: August 18, 2021 561-768-0700 www.tequesta.org The following agenda item is a Work Authorization for Kimley Horn and Associates to conduct an underwater inspection at Tequesta Bridge. The FDOT inspects the Bridge every two years. Transytems Corporation identified a potential soil leak at one of the Pilings, (6-4). The recommendation is to seal the joint. Additionally, minor above ground repairs were noted in the inspection report. The Consultant, Kimley Horn, will conduct and underwater inspection with two engineers with Scuba Certification. After which the consultant will provide a report of the their findings and repair recommendations. The cost associated with this work authorization is $4,800 and available in the Public Works Account 001.210.531-302, Engineering Services. To achieve this, three quotes were solicited from qualified vendors: American Painting, Sun Art Painting, and Munyan Painting. After meeting with Utilities staff to review the scope, three of the vendors submitted proposals (attached). Note: Munyan Painting originally declined to quote due to workload however they followed back up and chose to quote the work. The results are summarized below. Vendor Quote Notes American Painting Corp. $15,200 Quote meets all of the requested specifications for proper surface prep and painting. Sun Art Painting Corp. $26,500 Quote meets all of the requested specifications for proper surface prep and painting. Munyan Painting $10,729 Quote meets all of the requested specifications for proper surface prep and painting. After review of the proposals received, Utilities staff recommends awarding the project to Munyan Painting in the amount of $10,729. The approved FY21 budget included $15,000 to complete this project.
