ENGINEERING SERVICES WORK AUTHORIZATION
TEQUESTA DRIVE BRIDGE UNDERWATER INSPECTION SERVICES
This Work Authorization authorizes Kimley-Horn and Associates, Inc. to perform work set forth
herein and is issued pursuant to The Professional Engineering Services Agreement, between the
Village of Tequesta ("Client" or "Village") and Kimley-Horn and Associates, Inc. ("Kimley-Horn" or
"Consultant"), dated September 10, 2020 ("Agreement"). All terms and conditions of said
Agreement are hereby incorporated and made part of this Work Authorization.
PROJECT UNDERSTANDING
The Village received a bridge inspection report prepared by Transystems Corporation for FDOT
in November of 2020 for the Tequesta Drive Bridge.
The report identified a series of recommended minor above ground repairs. None of these repairs
are affecting the structural capacity or functional abilities of the bridge, but they do need to be
addressed to prevent potential future deterioration, tripping hazards, etc.
The report also identified a potential soil leak location in the underwater portion of the bridge
inspection report that requires further investigation to confirm the extent of the deficiency and the
required repair.
The Consultant will perform the following scope of work below, related to the investigation of the
underwater deficiencies.
SCOPE
Kimley-Horn will conduct a visual and tactile Level 1 inspection of the underwater wall area
identified in the Bridge Inspection Report (BIR). The visual and tactile Level 1 inspection will follow
the guidelines of American Society of Civil Engineers (ASCE) Practice No. 130 "Waterfront
Facilities Inspection and Assessment." The level 1 inspection will be conducted by two (2)
engineers with SCUBA Certification and a top -side spotter.
Upon completion of the underwater inspection, Kimley-Horn will provide a report of our findings
and repair recommendations to supplement the BIR. The report will be presented in letter format
with annotated photographs and sketches.
After submittal of the report, we will meet with Village representatives to discuss the timing of the
repairs and whether the repair details for all deficiencies need to be complied into a set of repair
plans.
ADDITIONAL SERVICES
Any services not specifically provided for in the above scope. as well as any changes in the scope
requested by the Village, will be considered additional services to this Work Authorization and will
be performed based on subsequent Work Authorizations approved prior to performance of the
additional services.
Potential additional services include the preparation of repair plans and bid documents to address
the bridge deficiencies.
INFORMATION AND SERVICES PROVIDED BY THE VILLAGE
Kimley-Horn assumes that all information provided by the Village can be relied upon in the
performance of professional services. The following information shall be provided to Kimley-Horn
and/or the following services will be performed by the Village.
Existing bridge records, plans and inspection reports (if there are any additional
documents)
Access to the Construction Site
SCHEDULE
Kimley-Horn will complete the work set forth herein within a mutually agreeable schedule based
on
COMPENSATION
Kimley-Horn will perform the services described in the Scope of Services above on a lump sum
basis for the amount of Four -Thousand Eight Hundred Dollars ($4,800.00).
