Regular Council STAFF MEMO Meeting: Regular Council - Aug 11 2022 Staff Contact: Merlene Reid, Director, HR & Risk Department: HR Management Approve Jupiter Medical Center Urgent Care 2022-2024 Renewal Agreement Addendum The Village requires an Occupational HealthCare agreement with a competent facility to provide occupational health services, to include candidate screenings, physicals, post -accident and random drug testing, and to assist in managing our workers compensation injuries from initial treatment through return to work status. A 2022-24 updated Contract Addendum for Services with amended Service Fees is attached as Exhibit A. The services listed are unique to departments and positions, with the two most popular ones being Drug Testing and General Physical. The former experienced no change, while the latter experienced a $10 increase. Located 6.4 miles from Village Hall, JMC Urgent Care is the closest occupational health facility offering the most comprehensive set of services and resources. Regular opening hours are wide-ranging from Monday to Saturday, 8:00am- 8:00p.m and Sundays, 9am-5 p.m., with Jupiter Medical Emergency room as an after-hours or emergency resource alternative. Continuity of service with JMC is also important in respect of readily available medical history and long-term employee treatment, especially as it relates to WC handling. ANNUAL RENEWAL ADDENDUM RENEWAL YEAR OCTOBER 1, 2022 through September 30,2024 This Annual Renewal Addendum (".Addendum") relates to that certain Agreement for Health Services (the "Agreement"} dated August 3, 2022 by and between Jupiter Medical Center Physicians Group, Inc. ("Prorr'der") and Village of Tequesta ("Company"). This Addendum must be signed by Company and returned to Provider on or before September 30,2022. In consideration of the mutual promises and covenants herein contained, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties hereto herby agree as follows: 1. The Agreement shall be renewed for the October I through September 30 Renewal Year set forth in the title of this document. 2. The Services and Fees Schedule on Exhibit A of the Agreement shall: a. REMAIN THE SAME AS THE PRIOR YEAR; or _X b. Be deleted in their entirety and replaced with the new Services and Fees set forth on Exhibit A attached herewith and incorporated into the Agreement by this reference. 3. Except as otherwise set forth in this Annual Renewal Addendum, the Agreement shall remain in full force and effect in accordance with its terms, IN WITNESS WHEREOF, in signing below, and intending to be legally bound thereby, the parties hereto hereby agree to the foregoing, which shall take effect as of the first day of the Renewal Year set forth in the title of this document.
