The Village of Tequesta's (VOT) Utilities Department (LID) needs to purchase various plumbing
and plumbing related tools. Broedell's Plumbing Supply, Inc., (Broedell's) located at 1601
Commerce Lane, Jupiter, FL, 33458, is located close to the VOT municipal limits. In order to
purchase supplies from Broedell's, a vendor credit application must be completed.
Attached is the required vendor credit application.
Funding for plumbing and plumbing tools is budgeted and available in the following accounts:

The UD recommends approval of the vendor credit application in order to use this vendor.

PUBLIC RECORDS. In accordance with Sec. 119.0701, Florida Statutes, CONTRACTOR must keep
and maintain this Agreement and any other records associated therewith and that are associated
with the performance of the work described in the Proposal or Bid. Upon request from the
Village's custodian of public records, CONTRACTOR must provide the Village with copies of
requested records, or allow such records to be inspected or copied, within a reasonable time in
accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR
who fails to provide the public records to the Village, or fails to make them available for
inspection or copying, within a reasonable time may be subject to attorney's fees and costs
pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida
Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated
with this Agreement or associated with the performance of the work described in the Proposal
or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and
following completion of the Agreement if the CONTRACTOR does not transfer the records to the
Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to
the Village, all public records in possession of the CONTRACTOR, or keep and maintain public
records required by the Village. If the CONTRACTOR transfers all public records to the Village
upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records
that are exempt or confidential and exempt from public records disclosure requirements. If the
CONTRACTOR keeps and maintains public records upon completion of the Agreement, the
CONTRACTOR shall meet all applicable requirements for retaining public records. Records that
are stored electronically must be provided to the VILLAGE, upon request from the Village's
custodian of public records, in a format that is compatible with the Village's information
technology systems.

Pursuant to Article XII of the Palm Beach County Charter, the Office of the Inspector General has
jurisdiction to investigate municipal matters, review and audit municipal contracts and other
transactions, and make reports and recommendations to municipal governing bodies based on
such audits, reviews, or investigations. All parties doing business with the Village shall fully
cooperate with the inspector general in the exercise of the inspector general's functions,
authority, and power. The inspector general has the power to take sworn statements, require
the production of records, and to audit, monitor, investigate and inspect the activities of the
Village, as well as contractors and lobbyists of the Village in order to detect, deter, prevent, and
eradicate fraud, waste, mismanagement, misconduct, and abuses.

The main man-made threat to sea turtles is artificial lighting near nesting beaches, which
effects both nesting females and legions of hatchlings. Even a single bright light near a nesting
site can cause all of the hatchlings on a given beach to head inland. Hatchlings have a finite
amount of energy when they hatch, which they desperately need to get to the water and swim
out to safety. Sea turtles are a "keystone species", which means they are an important part of
the environment and influence other species around them. Sea turtles have played vital roles
in maintaining the health of the world's oceans for more than 100 million years including
maintaining productive coral reef ecosystems and transporting essential nutrients, from the
oceans, to beaches and coastal dunes. Sea Turtle nesting season runs annually from May
through October.
The Village of Tequesta has been provided with a sea turtle display as part of the Village's
educational campaign regarding the protection of sea turtles which the Village can keep for up
to a year. In conjunction with the display, the Village is required to sign a simple one -page
contract (see attached) to ensure the longevity and preservation of the condition of the display.

Dear Lighting Display Lessee,
Sea Turtle Conservancy (STC) would like to ensure the longevity of the Sea Turtle Lighting
Display lending program by requesting your commitment to maintain the quality of our shared
displays. We ask the Town of Tequesta to take full responsibility in the upkeep of their lighting
display by regularly completing the following tasks during the continued leasing period:
o Assembling the display in an enclosed temperature -controlled environment that is
visible by staff and available to the public.
o Checking condition of all display parts, including fixtures, bulbs, and the monitor,
to ensure they remain in proper working order.
o Reporting major technical, mechanical or cosmetic damages to display surfaces,
display components, fixtures/bulbs and/or monitors during your continued lease,
including any damages incurred during transport, to STC.
o Maintaining a looped video play.
o Contacting STC to request replacement display components damaged due to
manufacturer error or technical failure.
o Lessees shall not alter lighting displays by customizing or altering components,
e.g. adding new parts, removing or replacing existing parts, altering the display
surface, or moving the display once it is assembled.
STC will not repair or replace display components damaged by lessee. All reports of damage to
the display and requests for display repairs or replacements should be directed to the STC
Lighting Display Program contact Casie Hain at (352) 373-6441 or casieneonserveturtles.org.
The Lessee will also assist with the coordination of transporting the display to the next host or
STC, if applicable. Coordination includes communication with STC and the next host,
disassembling the display and packaging it per the provided manual, and offering partial
transport to either STC staff or to staff from the next host facility if necessary.
STC and the Town of Tequesta will uphold this agreement during the duration of the leasing
period. The Lessee agrees to host the display as a continued lessee until STC may need the
lighting display back, which is an undetermined amount of time.

Treatment at Dover Ditch at a cost of $550 ($2200 Annually) with Additional Related
Services Available as Needed
The Village of Tequesta (Village) Utilities Department (UD) recommends approval of an annual
agreement with TCI ROOD for quarterly weed treatment at Dover Ditch. The services include all
labor, equipment and weed treatment solution. As can be seen in the agreement, quarterly
treatments cost $550 for an annual cost of $2200. UD is responsible for 60% of the costs for the
contract, so our actual spend will be $330 quarterly and $1,320 annually.
Through an interlocal agreement, stormwater runoff, drainage, and maintenance of Dover
Ditch is shared with the Town of Jupiter (TOJ). The TOJ is responsible for 40% of maintenance
expenses at Dover Ditch with the Village responsible for 60%. Other related services are
available in the agreement as needed.
Funding for weed control at Dover Ditch is budgeted and available in account 403-250-546.308

Additional Services (will be billed on a Time & Material basis per rates listed below):
Irrigation Repairs (labor) $70.00/1-ir + Parts (plus applicable Sales Tax)*
Plant Heath Care (labor) $70.00 Hr + Chemicals (plus applicable Sales Tax)
* If you are not a current irrigation maintenance customer, the initial inspection to the irrigation system will
include a $135.00 service call, which includes the first hour of labor, and our standard hourly rate of $85.00 per
man-hour thereafter. This inspection and possible repairs will be conducted on a Time and Materials basis.
The following services can be performed and billed on a Time & Material basis:
• Landscape enhancements
• Pruning of trees or privacy hedges (over 8 feet in height) or espalier of trees or vines
• Irrigation renovations
• Irrigation Repairs
• Installation of mulch or soil in planting beds or other areas
• Citrus program which includes spraying for fungus, insects, and a foliar spray for nutritional value, (note there
is no control for citrus canker or citnis greening)
• Deep Root Feed Palms which includes a liquid drench of fertilizer applied to the soil around the root mass of
all specialty palms
• Tetracycline Injections which includes injections for coconuts to help prevention of lethal yellowing
• Hibiscus Care which includes a growth inhibitor sprayed every two (2) months to maintain Dwarf Hibiscus
• Post -storm cleanup
• Ficus - White fly and Spiraling White fly treatments
• Quarterly bud drenches for all "Canary Island Date Palms" to aid in the prevention of palm weevils
• Lawn and Plant disease treatment
• Suppression of sedge grasses in turf area
EXCLUSIONS
The following are not included in any program: roses, orchids, annuals, vegetables, fruit bearing
trees, sabal palms, large established trees (8" trunk diameter), nematodes, palmetto weevils, boring
insects, termites or other wood destroying insects, take all patch, perennials, eastern lubber
grasshoppers, anny worms, PH and nutrient corrections, grassy weed control including but not
limited to crabgrass, carpet grass and bermuda grass control, soil borne diseases and moles.
MAINTENANCE CONTRACT CONDITIONS
A.) Advanced notice shall be given for any extra work when conditions permit.
B.) All work is to be performed in a professional manner, according to industry practices. Services shall
be rendered as specified with the exception of legal holidays, strikes, accidents, inclement weather, natural
disasters, terrorist attack, pandemic, or any other delays beyond the control of Rood Environmental.
C.) Rood Environmental shall provide all labor (dressed in company uniform), with supervision,

equipment and materials necessary for the performance of this agreement.
D.) Rood Environmental agrees to maintain a general public liability insurance policy as required bylaw
and Worker's Compensation insurance. A copy of our current Certificate of Insurance is available upon
request.
E.) Rood Environmental is a drug free workplace, and agrees to continue this program.
F.) Any repairs or extra work necessary, due to hurricanes, floods, lightning or other acts of nature, shall
not be covered under this agreement.
G.) Thi,, agreement may be cancelled by either party upon thirty (30) days written notice.
H.) It is mutually agreed upon by both Rood Environmental and the Client that this Maintenance
Contract will be in effect for a twelve-month period (I -year). At contract end date, if neither party cancels
or gives notice of canceling agreement; the contract will annually auto -renew for the next twelve-month
period (I year). Notification of rate changes, if applicable, may be sent for renewal period.
I.) Client acknowledges that Contractor has made a significant investment in the recruitment, hiring, and
training of TCIROOD staff. Therefore, client and/or it's agents agrees not to solicit, recruit hire or
otherwise engage the services of any TCIROOD employee without the prior written consent of
TCIROOD.

PUBLIC RECORDS. In accordance with Sec. 119.0701, Florida Statutes, CONTRACTOR must keep
and maintain this Agreement and any other records associated therewith and that are associated
with the performance of the work described in the Proposal or Bid. Upon request from the
Village's custodian of public records, CONTRACTOR must provide the Village with copies of
requested records, or allow such records to be inspected or copied, within a reasonable time in
accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR
who fails to provide the public records to the Village, or fails to make them available for
inspection or copying, within a reasonable time may be subject to attorney's fees and costs
pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida
Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated
with this Agreement or associated with the performance of the work described in the Proposal
or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and
following completion of the Agreement if the CONTRACTOR does not transfer the records to the
Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to
the Village, all public records in possession of the CONTRACTOR, or keep and maintain public
records required by the Village. If the CONTRACTOR transfers all public records to the Village
upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records
that are exempt or confidential and exempt from public records disclosure requirements. If the
CONTRACTOR keeps and maintains public records upon completion of the Agreement, the
CONTRACTOR shall meet all applicable requirements for retaining public records. Records that
are stored electronically must be provided to the VILLAGE, upon request from the Village's
custodian of public records, in a format that is compatible with the Village's information
technology systems.

119, FLORIDA STATUTES, TO CONTRACTOR'S DUTY TO PROVIDE PUBLIC RECORDS

Pursuant to Article XII of the Palm Beach County Charter, the Office of the Inspector General has
jurisdiction to investigate municipal matters, review and audit municipal contracts and other
transactions, and make reports and recommendations to municipal governing bodies based on
such audits, reviews, or investigations. All parties doing business with the Village shall fully
cooperate with the inspector general in the exercise of the inspector general's functions,
authority, and power. The inspector general has the power to take sworn statements, require
the production of records, and to audit, monitor, investigate and inspect the activities of the
Village, as well as contractors and lobbyists of the Village in order to detect, deter, prevent, and
eradicate fraud, waste, mismanagement, misconduct, and abuses.
