Three sealed bids were submitted by the 2:00 p.m. deadline.• One additional bid was received by fax rather than in a sealed envelope as required and, therefore, we recommend that it be rejected. - :All three of the bidders submitted bids for the Seabrook Road project, however, only two bids were received for.Country Club Drive Phase III. One'of the bidders chose to offer a 2.5% reduction in the total bid amount if selected for both projects. 'The reduction amounts to approximately$5,000.00. , • :-As'indicated-on the attached bid tabulation;:the low.bid:for.both.projects (individually and combined)was submitted by.Green:Acres.LN:Service;_Inc.;.a.landscape-constniction firm • located in Fort Lauderdale. Their bid is as follows: • A. Country Club Drive Phase III S120,533.00 B. Seabrook Road Streetscape $ 74,035.00 C. Country Club Drive Phase III and Seabrook Road Streetscape S189,443.00 • The combined bid submitted for both projects is S 189,443.00. Several matters regarding this bid should be noted as follows: 1. Green Acres does not hold a Certified Landscape Contractors License as required by the bidding documents. The firm also does not have a certified arborist or pesticide • applicator as required. 2. The time period indicated by Green Acres for completing both projects is 150 days or 5 months.. 3. The bid cost breakdown indicates a total amount of$4,500.00 for the one-year maintenance and warranty item for both projects. Green Acres advises that this figure is in error and should have been doubled for a total of S9,000.00, however, the firm will abide by the lower figure. The firm holds a Certified Landscape Contractors License and has the services of a certified arborist and pesticide specialist. In addition to the bid of Green Acres, two bids were received for the Seabrook Road Project. The lower of these bids was submitted by Terracon Services, Inc. and is $75,904.00. This bid is $1,869.00 or 2.5% more than the low bid of Green Acres. The other bid, submitted by Aiello is $10,765.00 or nearly.15%more than the low bid. .Terracon holds a Certified Landscape .Contractors License and has the services of a certified arborist and pesticide specialist. Analysis and Recommendation • • Unless the Village was to waive the requirement that bidders must hold a Certified Landscape •. Contractors License, the bid submitted.by Green.Acres must be rejected. The requirement has been imposed by the Village on all recent landscape-related projects at time of bidding and is considered very desirable in assuring competent services. That view is shared by the designers of the two projects in question. It is, therefore, recommended that the Country Club Drive project be awarded to Aiello Landscape, Inc. by acceptance of its bid $126,092.00.:.The projected construction timeframe is indicated as 90 days. . It is further•recommended that_the-Seabrook.Road.project be.awarded.to Terracon Services,Inc. • .. by acceptance.of its bid of$75,9.04:00: .:The.projected construction•timefrarne is 45:days. -.This ::bid is:approximately;11%-less than the Aiello..bid.for.the:project and:offers a:25.%reduction in the construction timeframe indicated by Aiello. Only one alternate bid•was received for substituting larger oak trees for those specified in the base bid. The cost of that alternate bid submitted by Terracon for the Seabrook Road project added approximately$13,000.00 or 18%to the Seabrook Road project. No alternate bid was submitted for Country Club Drive. Availability of quality trees at the larger size is limited and the quality of those trees may be marginal. The additional cost does not appear justified and the alternate is, therefore, not recommended. The recommendations are based on approval by the Village attorney, that insurance and bonding requirements are met. EINAND, FIRE CHI � SUBJECT: REVIEW OF EMS TRANSPOR S DATE: 1/16/01 In accordance with Ordinance No. 523, Section 9-54, Determination of Fee Amounts, and the directions given by the Public Safety Committee to review the EMS transport charges annually,. have attached a spreadsheet denoting the surrounding advanced life support providers and their current fee schedule for ALS transports. For comparison purposes, I have included a column titled Four Mite Transport. This column was added to denote the total fee associated with a four- mile transport,which represents approximately 95%of our transports in the Tequesta area. As you can see from this spreadsheet, the average advance life support transport rate, with the other associated charges is $323.75 and the average mileage rate is$6.52 per mile, making the average four-mile ALS transport$349.83. Therefore, I am recommending that our EMS transport rate be adjusted to $320.00 per transport, and the mileage rate be adjusted to $6.50 per mile. These new rates would denote a$346 transport charge from Tequesta to Jupiter Medical Center, which is just under the average of the survey: This also equates to.a 1.76% rate increase over this years ambulance rates. When this service was adopted by Ordinance, the Ordinance gave the ability to adjust the rates on 102 annual basis by resolution. Therefore, I have attached a resolution for the Village Council's coritderation. If you see it fit, please forward this to the February 2001 Village Council agenda for.tleir'consideration, action or comments. r - I would also like the Village Council to consider adding an ALS Level 2 user fee. This level 2 charge is important to maximize the revenues allotted under the Medicare Reform Act and helps offset the cost of additional medications given in critically injured patients. For example,we are in the process of revising our medical protocols to include the new medication called Amiodarone (see attached article). Tequesta Pharmacy has quoted us a price of $90.00 per 3mg. A 3mg dose is what is administered to patients requiring this type of treatment. This medicine is primarily used in critically injured patients (usually Cardiac Arrests) and studies have shown that this drug has increased the survival rate by 27 percent, when utilized. To the best of knowledge, no other EMS provider in this area is using this medication; Tequesta will again be the leader in pre-hospital emergency care... During the negotiated rule-making cession of the Medicare Reform Act, ambulance providers and the International Association of Fire Chiefs were successful in demonstrating to the Medicare Officials that critically injured patients cost more to treat than what is currently being reimbursed • under the existing fee schedule. Medicare agreed to an ALS Level 2 rate structure and would reimburse at a higher rate if the agency had a higher rate charge, if not they will reimburse at the 41) ALS level 1 rate. I recommend that we establish an ALS Level 2 treatment rate of $465.00 and $6.50 per mile. This will help offset the cost of this new treatment and maximize the insurance reimbursement revenues available. An ALS level 2 charge is determined by the following Medicare definition; Advanced Life Support, Level 2: Where medically necessary, the administration of at least three different medications and/or the provision of one of the following ALS procedures: Manual defibrillation/cardioversion, Endotracheal intubation, Central venous line, Cardiac pacing, Chest decompression, Surgical airway, or Intraosseous line. Another factor to keep in mind is that it is always prudent to keep our transport and mileage rates above the current Medicare allowable, to maximize future revenues.
