WHEREAS, TFR and Life Scan have entered into a Professional Services Agreement (hereinafter "1'S A") to provide annual Life Scant Firefighter Physicals that are NFPA 1582 and NFPA 1553 compliant per Scope of Services Attachment A, WHEREAS, The PSA shall be for a term of.two (2) years commencing on the I"day of Feb. 2024 and shall renew for two (2)additional one(1)year terms,with annual 5%increases. WHEREAS,the PSA provides that TFR will compensate Life Scan for each Firefighter Physical performed per Life Scan Pricing Attaclxnent A.. WHEREAS, the PSA provides that Life Scan'and the TFR will mutually agree to a time and place to perform the Life Scan Firefighter Physicals and that TFR will provide the location for the on-site physicals.
