Rescue
Please find attached Host Agreement between Dive Rescue International and Tequesta
Fire Rescue for Swiftwater Rescue Combo Session to be held June 2-6, 2025. The
preferred class size is 20 participants. Both parties agree to abide by the terms and
conditions of the agreement. The final date when the class may be canceled with no
financial obligation to Dive Rescue International is May 12, 2025. The associated fees
are as follows:
• Tuition per student per course: $1,000
• Upon the registration of 15 paid participants, the hosting agency may enroll up
to 5 participants at no charge.
• A minimum of 10 paid tuitions will be required for this course to proceed as
scheduled.

HOST AGREEMENT
This is an agreement made on March 19,2025 between Dive Rescue International and the Tequesta Fire
Rescue. We agree,and our agencies agree,to be the host for the training course entitled Swiftwater Rescue
Combination to be held inTequesta,FL on June 2-6,2025.
1 understand Dive Rescue International will provide the following:
+ Qualified Instructor or Instructors.
•:• Registration of students and collection of tuition.
❖ Instructor fees,travel,and expenses.
❖ Custom training materials and/or textbooks.
•:• Specialized training equipment as required for the course.
❖ Customized brochure,marketing material and support.
•:• Screening of participants to ensure membership with a public safety agency.
•:• Instructor transportation to and from the airport and during his/her stay.
ee Certificate of Completion awarded to students performing at an acceptable level.
•:• A one-time 10% discount for equipment purchased from Dive Rescue International. This
purchase must be made within 90 days of the class and applies to current retail prices.
Host responsibilities:
•:• Complete and return all paperwork.
ee Act as a contact for inquiries concerning local information.
•:• Promote and publicize the course to other public safety agencies.
•: Arrange for all instructional facilities which include classroom,audio/visual equipment,and training
sites,as outlined in the Program Information Guide.
I understand that the preferred class size for the Swiftwater Rescue Combination course is 20
participants. The tuition for this course is $1000(US Funds) per student. A minimum of 10 students is
required for this class to proceed as scheduled.
I understand that if I cancel the scheduled courses by May 12,2025, 1 will have no financial obligation to
Dive Rescue International. However, if cancellation is made after this date, I agree to compensate Dive
Rescue International for any non-refundable preparation,shipping or travel expenses.

Re: Consider Approval of Holtz Consulting Work Authorization for WM#1 Tequesta Drive
Water Main (WM) Replacement Project for a Total Not to Exceed $11,665
The Village of Tequesta (Village) Utilities Department (UD) recommends approval of work authorization
(WA) 16 with Holtz Consulting for"WM#1 Tequesta Drive Water Main Replacement" project (Project).
The work authorization includes engineering services for the design update and bidding phase services
for a project to replace approximately 5,600 feet of existing asbestos cement(AC) potable water main.
This work authorization total is not to exceed $11,665. This project was 100% complete and was
originally bid together with the WM#4 Country Club Dr S water main replacement project in November
2022.The bids for the combined project came in at 60% higher than the engineer's opinion of probable
construction cost (OPC).Therefore,the WM#1 portion was deferred due to a lack of funding. Due to the
implementation of the capital charge in 2023, we can now fund it as a pay-go project.
Streets included in the Project:
• El Portal Drive from Yacht Club Place to Country Club Drive
• Tequesta Drive from County Club Drive to Pinetree Drive
• Tequesta Drive from the Tequesta Bridge to Fiesta Drive
This Project is part of the phased AC water main replacement program included in the "May 2020 Village
of Tequesta Facilities Assessment Update," developed for the Village UD by Kimley-Horn. Attached is a
map of the planned AC pipe replacement projects.
At the March 3, 2025,Village Council workshop,the UD presented its updated financial projections,
provided by the Village's Finance Department, which showed that implementing the 2023 capital charge
as a part of the rate increase has allowed us to catch up financially in revenues. Based on this, the UD is
prepared to proceed with two additional capital projects on a cash basis.The new recommended project
schedule includes moving the SA Well 19 Replacement project and WM#1 Tequesta Dr. water main
replacement forward this year, which were deferred in 2022 due to lack of funding.

(CCNA), FS287.055, approved on September 10, 2020.The CCNA requires public agencies to select
professional service firms based on qualifications.

PROJECT TITLE: WM #1 Tequesta Dr. Water Main Replacement Project
This Work Authorization, when executed, shall be incorporated in and shall become an integral
part of the Agreement for Professional Services dated September 10, 2020 between the Village of
Tequesta ("VILLAGE"), and Holtz Consulting ("CONSULTANT") hereafter referred to as the
Agreement.
I. PROJECT DESCRIPTION
The project will include engineering services for the design update, bidding, and construction of
approximately 5,600 feet of potable water main to replace the existing asbestos cement (AC) pipe
along the streets listed below:
• E1 Portal Drive from Yacht Club Place to Country Club Drive
• Tequesta Drive from Country Club Drive to Pinetree Drive
• Tequesta Drive from the Tequesta Bridge to Fiesta Drive
The existing AC pipe was installed in the 1950's and 1960's and has reached the end of its useful life.
Replacing the AC pipe will reduce the risk of failure and increase the resiliency of the Village's potable
water distribution system. Included in the design will be the replacement of water services from the
new water mains to the existing water meters, new fire hydrants, and the grouting/abandonment of the
existing AC pipe after the new water mains are placed into service.

II. SCOPE OF SERVICES
The following is a description of the services to be provided under this Work Authorization.
TASK 1 —DESIGN SERVICES
CONSULTANT will update the existing design plans previous prepared under a past Work
Authorization to incorporate water main improvements that have already been completed under the
previous project and to meet the new asphalt milling and overlay requirements of the Village Public
Works Department. CONSULTANT will also update the plans to include adding swales as part of
the restoration work where the new water main is installed via open-cut methods.
CONSULTANT shall provide draft updated drawings and technical specifications for VILLAGE
review. Technical specifications will be based on the CSI 16 division format. The front-end contract
documents provided by the VILLAGE will be incorporated into the Contract Documents by the
CONSULTANT.
Following draft submittal review by the VILLAGE, CONSULTANT shall meet with VILLAGE to
discuss comments,and then prepare the documents incorporating VILLAGE'S comments for the final
set of construction documents.
CONSULTANT shall also update the engineer's opinion of probable cost of construction. A blank
and completed bid form will be prepared and submitted to the VILLAGE for their insertion into the
VILLAGE's front-end bidding and contract documents.
TASK 2—BIDDING/PROCUREMENT SERVICES
CONSULTANT will assist the VILLAGE with advertising and bidding the project or procuring a
contractor by "piggy-backing" an existing public construction contract.
Under this task CONSULTANT may perform a combination of the following activities:
• Prepare bid advertisement and provide bid documents to bidders.
• Conduct a pre-bid meeting and issue meeting minutes to all plan holders.
• Conduct a site visit/walkthrough with potential contractor(s).
• Respond to questions bidders may have concerning the bid documents.
• Issue addenda as necessary to clarify the bid documents.
• Attend bid opening and prepare bid tabulation form.
• Evaluate bids and prepare recommendation of award to the lowest responsive, responsible
bidder.
• Prepare Final Contract Documents
• Prepare and review executed contract documents.
• Prepare Contractor's Notice to Proceed.

V. COMPENSATION AND PAYMENT
The CONSULTANT will perform the tasks as outlined in the scope of services for Lump Sum Fee
Of $11,665.00 in accordance with our approved billing rates with an estimate of costs by task
provided in Table 2 and further detailed in Attachment A. Monthly progress payments will be
authorized based on percent complete as determined by CONSULTANT and approved by
VILLAGE.

VI. ASSUMPTIONS
1. The design drawings (including survey, subsurface exploration, etc.)will be based on the
plans previously prepared and bid with the AC Water Main Replacement Project#4. No
updated survey or subsurface investigation is included.
2. The new water main sizes will be determined by the VILLAGE. No hydraulic modeling
or fire-flow modeling is included in this proposal.
VII. AGREEMENT REFERENCE
The work authorized under this Work Authorization shall be performed under the terms and
conditions described within the Professional Services Agreement dated September 10, 2020, by and
between the Village of Tequesta ("VILLAGE") and Holtz Consulting ("CONSULTANT").
Compensation shall not exceed the hourly rates currently in effect under this Agreement.

PUBLIC RECORDS. In accordance with Sec. 119.07011 Florida Statutes, CONTRACTOR must keep
and maintain this Agreement and any other records associated therewith and that are associated
with the performance of the work described in the Proposal or Bid. Upon request from the
Village's custodian of public records, CONTRACTOR must provide the Village with copies of
requested records, or allow such records to be inspected or copied, within a reasonable time in
accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR
who fails to provide the public records to the Village, or fails to make them available for
inspection or copying, within a reasonable time may be subject to attorney's fees and costs
pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida
Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated
with this Agreement or associated with the performance of the work described in the Proposal
or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and
following completion of the Agreement if the CONTRACTOR does not transfer the records to the
Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to
the Village, all public records in possession of the CONTRACTOR, or keep and maintain public
records required by the Village. If the CONTRACTOR transfers all public records to the Village
upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records
that are exempt or confidential and exempt from public records disclosure requirements. If the
CONTRACTOR keeps and maintains public records upon completion of the Agreement, the
CONTRACTOR shall meet all applicable requirements for retaining public records. Records that
are stored electronically must be provided to the VILLAGE, upon request from the Village's
custodian of public records, in a format that is compatible with the Village's information
technology systems.

Aquifer Well 19 Replacement—Final Design and Bid Phase Services for a Total Not to
Exceed $21,000
The Village of Tequesta (Village) Utilities Department (UD) recommends approval of a work
authorization (WA)with Mock Roos and Associates (MRA) for the final design and bid phase services for
the replacement of surficial aquifer(SA) well 19 (SA Well 19 Replacement), with a total not to exceed
$21,000.The SA Well 19 project is 99% designed, permits are in place, and it was scheduled to be bid in
FY22. Due to the implementation of the capital charge in 2023,we can now fund it as a pay-go project.
SA Well 19 was one of the wells recommended for replacement in the July 2020"Wellfield Condition
Assessment" report and needs to be replaced due to the deterioration of its mechanical components
and piping, to preserve its production capacity and capabilities.The wellhead and equipment are heavily
corroded and do not meet current regulatory requirements. It is located at Tequesta Park and is one of
three wells that have an access vault for the mechanical components below ground, making
maintenance more difficult and time-consuming, and increasing the susceptibility to bacteriological
failures during lab testing.
The well was constructed in 1975, demonstrating a lifespan of 50 years to date,which is comparable to
the average lifespan of wells, typically ranging from 20 to 50 years. Well longevity varies depending on
many factors and can last far longer, especially when proper mechanical equipment and components
are in place and regular maintenance is conducted.
SA well 19 is part of the Village's system of 11 surficial aquifer wells, as permitted in our water use
permit(WUP) No. 50-00046-W, issued by the South Florida Water Management District (SFWMD). Our
SFWMD WUP allows for the withdrawal of 401 million gallons (MG) per year from the surficial aquifer.
At the March 3, 2025, Village Council workshop,the UD presented its updated financial projections,
provided by the Village's Finance Department, which showed that implementing the 2023 capital charge
as a part of the rate increase has allowed us to catch up financially in revenues. Based on this, the UD is
prepared to proceed with two additional capital projects on a cash basis.The new recommended project
schedule includes moving the SA Well 19 Replacement project and WM#1 Tequesta Dr. water main
replacement forward this year, which were deferred in 2022 due to lack of funding.

public agencies to select professional service firms based on qualifications.

Please note: The terms used in this Work authorization with initial capital letters have the meanings
stated in the original "Agreement for Professional Services" dated September 10, 2020, between
the Village of Tequesta and Mock, Roos, & Associates, Inc., or in the "Engineers Joint Contract
Documents Committee's (EJCDC) standards.
PROJECT TITLE: Well SA 19 Replacement—Final Design and Bid Phase Services
This Work Authorization, When executed, shall be incorporated in and shall become an integral
part of the Agreement for Professional Services dated September 10, 2020, between the Village of
Tequesta ("VILLAGE") and Mock, Roos & Associates, Inc. ("CONSULTANT"), hereafter
referred to as the Agreement.
I. PROJECT DESCRIPTION
The VILLAGE has a water use permit (No. 50-00046-W) from the South Florida Water
Management District(SFWMD) that allows the withdrawal of 401 million gallons (MG)
per year from the surficial aquifer and 1,251 MG from the brackish upper Floridan aquifer
(UFA).
In 2021, the CONSULTANT completed the design for the replacement of Well No. 19,
followed by the permitting in 2023. Although the project Was placed on hold, the
VILLAGE is now prepared to move forward with updating the drawings, finalizing Florida
Power and Light (FPL) coordination, and advertising the project for bid. No significant plan
revisions are anticipated before the VILLAGE proceeds with the bid advertisement.
II. SCOPE OF SERVICES
The following is a description of the anticipated services to be provided under this Work
Authorization.

TASK I —FINAL DESIGN PHASE
I Attend a site visit to Well No. 19 to review the existing site conditions compared to
the proposed 2023 construction drawings.
2. Update the drawings with the current Village Council, current date, and update
references to current codes where applicable. Make minor revisions to the drawings
based on current standards. Confirm that the FEMA FIRM map elevation does not
require the well to be elevated.
3. Prepare an updated "Engineer's Opinion of Probable Construction Cost".
4. Prepare Technical 'Specifications, including Bid Advertisement, Instruction to
Bidders, and Schedule of Values.
5. Provide the professional services of a hydrogeological subconsultant and an electrical
subconsultant to assist with the tasks above. Coordinate senlices of all
sub consultants.
TASK 2 - BID PHASE
I. Attend a pre-bid meeting.
2. Issue appropriate addendurns responding to questions from the.prospective bidders, if
any, during the bid period.
3. Review the qualifications of the apparent low bidder and provide the VILLAGE with
a"Recommendation of Award"' from an engineering perspective based on bids
received.

*This fee estimate is based on specific labor classifications but may include other approved
classifications per the Master Agreement with MockeRoos. Task fees are for reference only,
as the project is billed time and expenses (subconsultants are lump sum expenses). While
the total fee will not be exceeded without prior approval, MockeRoos invoices will reflect
actual hours worked rather than individual task estimates.
ASSUMPTIONS
I. Any services not included in the Scope of Services will be considered Additional. Services.
2. Additional Services can be provided upon CONSULTANT receiving signed authorization
from the VILLAGE.
3. Project deliverables will generally be .mailed PI F files of the drawings, specifications,
and bid documents.
4. One (1) Bid is anticipated and included in this proposal.
5. The VILLAGE will be responsible for advertising the bid, printing and distributing bid
documents (either electronically or in hard copy), and receiving and time-stamping
submitted bids. Received bids will be scanned and provided to CONSULTANT to review

6. The VILLAGE owns the property or has an easement for the proposed well. A title search.

The Village of Tequesta Utilities Department (UD) recommends approval of a work authorization (WA)
with Mock Roos and Associates (MRA)for Surficial Aquifer(SA)well 26 performance evaluation and
monitoring, including historical well 26 data review, performance and water quality field testing, and
static water level monitoring for three days meetings,for a total not to exceed $9,800.
In January 2025, SA well 26 was rehabilitated and video-logged by AMPS due to decreased specific
capacity, water quality, and increased air production in the well, causing discolored water.This well has
traditionally been a very dependable well and the recent well production characteristics rendered this
well less favorable for operation. Internal investigations have not indicated the cause of the entrained
air. Engaging a hydrogeologist is essential for a detailed assessment of the well's characteristics and
resolution(s). As subject matter experts, they will leverage their expertise and specialized knowledge to
thoroughly investigate the underlying issues and recommend effective solutions. Please see the
attached scope of services for more details.
MRA has a master agreement with the Village through the Consultant's Competitive Negotiations Act
(CCNA), FS287.055, approved on September 10, 2020.The CCNA requires public agencies to select
professional service firms based on qualifications.
The Water Fund account will fund the work authorization for professional engineering services.

11. SCOPE OF SERVICES
The following is a description of the anticipated services to be provided iiiider this Work
Authorization. The CONSULTANT Nvill provide the services of JLA Geosciences., Inc.,, a
hydrogeologist subconsultant to assist with the tasks below.
TASK I — WELL 26 PERFORMANCE EVALUATION AND MONITORING
Historical Well 26 Data Review
The CONSULTANT will review available historical wellfield data for Well 26, including:
Well Construction aiid Testing Reports
o As-built diagrams and well construction details
o Previous Connect Consulting wellfield testing data for Well 26
0 Village operating data and notes, if available
Well Rehabilitation Work by AMPS
(-) AMPS Well 26 performance testing reports

This task will include discussions with the VILLAGE'S operations staff to review any
additional well data and a site visit to Well 26 and the adjacent irrigation well. The
CONSULTANT will provide a summary letter via an email, including findings and
recommendations to improve water quality issues.
Well 26 Performance and Water Quality Field Testing
CONSULTANT will perform xvater level and performance testing on Well 26. Testing will
include:
a Static and pumping water levels measured for tip to two (2) hoLirs
0 PL1111ping rate and pressure measurements using existing well equipment
0 Field water quality testing parameters, including: Specific capacity, Sand content,
Ttirbidity, Hydrogeii sulfide, Total arid soluble iroii, pH, Dissolved oxygen,
Temperature, Specific conductance, etc.
Additional parameters recorded will include static and pumping water levels, pumping
rates, putiiping pressure, and general field observations. Upon completion of testing, the
CONSULTANTwill summarize the testing data in table format and list identified well
performance issues, SLICII as undesirable water quality or Linde rperfo 1-mall c e. A letter
summarizing the findings will be provided via email following the fieldwork.
Well 26 Static Water Le-vel Monitoring (3 Da rs)
If the wellhead configuration allows, the CONSULTANT will install a pressure transducer
and data logger in Well 26, below the static water level, to monitor- water levels over a

three-day period. This monitoring will assess the influence of adjacent wells, including the
nearby irrigation well, on Well 26.
• Well 26 must remain idle and not be operated during the monitoring period.
• A CONSULTANT hydrogeologist will visit the well once per day to manually
measure water levels and verify data logger readings.
• Upon completion of the monitoring period, the CONSULTANT will provide a
letter via Email including a data chart summarizing the findings within one week
following the fieldwork.
Assumptions:
1. The VILLAGE will provide all available historical data, including well construction
details,, operating records, and rehabilitation reports for Well 26. The Village will also
provide relevant information regarding, the nearby irrigation well, including installation
details and operational data if available.
The Village will grant the CONSULTANT access to Well 26 and the adjacent irrigation
well for site visits, testing, and monitoring. The Village will provide staff and equipment
for access to the well and operation of the well to complete tithe scope of work. Field
work will be scheduled at a mutually beneficial time.
3. Well 26 must be idle and not operated during the three-day static water level monitoring
period. The existing well equipment must be functional and available for performance
testing.
4. Installation of a pressure transducer and data logger is contingent upon the wellhead
configuration allowing for equipment placement. If installation is not feasible, an
alternative monitoring approach may be proposed as additional services.
5. Field water quality testing will include parameters listed in the scope of work-, laboratory
analysis is not included unless specifically requested as an additional service.
Performance testing duration is limited to two (2) hours.

PUBLIC RECORDS. In accordance with Sec. 119.07011 Florida Statutes, CONTRACTOR must keep
and maintain this Agreement and any other records associated therewith and that are associated
with the performance of the work described in the Proposal or Bid. Upon request from the
Village's custodian of public records, CONTRACTOR must provide the Village with copies of
requested records, or allow such records to be inspected or copied, within a reasonable time in
accordance with access and cost requirements of Chapter 119, Florida Statutes. A CONTRACTOR
who fails to provide the public records to the Village, or fails to make them available for
inspection or copying, within a reasonable time may be subject to attorney's fees and costs
pursuant to Sec. 119.0701, Florida Statutes, and other penalties under Sec. 119.10, Florida
Statutes. Further, CONTRACTOR shall ensure that any exempt or confidential records associated
with this Agreement or associated with the performance of the work described in the Proposal
or Bid are not disclosed except as authorized by law for the duration of the Agreement term, and
following completion of the Agreement if the CONTRACTOR does not transfer the records to the
Village. Finally, upon completion of the Agreement, CONTRACTOR shall transfer, at no cost to
the Village, all public records in possession of the CONTRACTOR, or keep and maintain public
records required by the Village. If the CONTRACTOR transfers all public records to the Village
upon completion of the Agreement, the CONTRACTOR shall destroy any duplicate public records
that are exempt or confidential and exempt from public records disclosure requirements. If the
CONTRACTOR keeps and maintains public records upon completion of the Agreement, the
CONTRACTOR shall meet all applicable requirements for retaining public records. Records that
are stored electronically must be provided to the VILLAGE, upon request from the Village's
custodian of public records, in a format that is compatible with the Village's information
technology systems.

Pursuant to Article XII of the Palm Beach County Charter, the Office of the Inspector General has
