PREVENTIVE MAINTENANCE AGREEMENT
THIS CONTRACT is made and entered into on May 22, 2026 , by and between
Quick Fitness Repairs LLC (the "Service Company"), whose address is 3504 W 94tn
Street Hialeah 33018 and Insert Property: Tequesta Police Department located at
Insert Address: 357 Tequesta Drive Tequesta, Florida
WHEREAS Service Company is a company engaged in the business of servicing and
maintaining Fitness Equipment and is willing to provide such services to Customer as
per the terms herein.
Services Provided
Effective the date of this Agreement Quick Fitness Repairs will perform the Preventive
Maintenance (PM) of cardiovascular and strength services described herein, (4) times per
year through its own work force.

a. Test cardiovascular equipment for proper operation.
b. Clean cardiovascular equipment per manufacturer recommendation.
c. Inspect treadmill belts and decks for signs of wear.
d. Remove treadmill motor shrouds and vacuum internally.
e. Lubricate treadmill belts and decks per manufacturer recommendation.
f. Ellipticals inspect mechanical parts for signs of wear.
g. Check bike chains, cranks, pedals, and straps for signs of wear.
h. All machines will be serviced on each visit.
i. Inspection and general maintenance of all strength training equipment, including
but not limited to: adjusting cables, belts, pulley alignment, tightening bolts and
adjusting range of motion cams
Repairs
Requests for repairs resulting from the preventative maintenance (PM) or service calls
will be attended to within 24-48 hours. The cost of servicing calls will be charged at a
rate of $175.00 for the service call. In the event that Original Equipment Manufacturer
(OEM) parts are not accessible, the service provider will make use of parts that meet
the manufacturer's standards for original equipment.
Term, Fee, and Payment
The Agreement can be ended by either party, with or without reason, by providing a
written notice of thirty (30) days. The renewal of the Agreement is performed automatically
on an annual basis, unless a written notification is provided thirty (30) days before the end
of the term.

All payment terms shall be Net 30. We accept checks, Visa, MasterCard and American
Express, and Zelle. A receipt will be provided if requested.
All payments are to be sent to Quick Fitness Repairs 3504 W 94th Street Hialeah 33018
Taxes
Any and all applicable taxes shall be added to any of the charges specified.
Insurance Disclaimer Indemnification
Each party affirms to the other that it has obtained all legally required insurance for its
employees, equipment, and operations. This agreement solely pertains to service and
Servicer, including its owners, directors, officers, employees, and agents, shall not be
held responsible for any liability arising from the use of the serviced equipment or the
condition and use of the premises where the equipment is located. In relation to this, the
Customer agrees to indemnify and protect Servicer, its owners, directors, officers,
employees, and agents from any and all legal claims, lawsuits, loss, costs, damages,
liabilities, and expenses, including legal fees (in or out of litigation, and for any appeals),
resulting from the condition or use of the equipment and/or the premises in which it is
situated by any person.

Miscellaneous Provisions
(a) Applicable Law: This Contract shall be construed under and in accordance with the
laws of the Florida, and all obligations of the parties created under this Contract are
performable in Miami Dade County, Florida.
(b) Parties Bound: This Contract shall be binding on and inure to the benefit of the
parties to this Contract and their respective heirs, executors, administrators, legal
representatives, successors and assigns as permitted by this Contract.
(c) Legal Construction: In the event any one or more of the provisions contained in this
Contract shall for any reason be held invalid, illegal, or unenforceable in any respect,
that invalidity, illegality, or unenforceability shall not affect any other provision. This
Contract shall be construed as if the invalid, illegal, or unenforceable provision had
never been contained in it.
(d) Prior Contracts Superseded: This Contract constitutes the sole and only Contract of
the parties and supersedes any prior understandings or written or oral Contracts
between the parties respecting the subject matter of this Contract.
(e) Amendments. This Contract may be amended by the parties only by a written
Contract.
(f) Attorneys' Fees: If any action at law or in equity is brought to enforce or interpret the
provisions of this Contract, the prevailing party will be entitled to reasonable attorneys'
fees in addition to any other relief to which that party may be entitled.

Signatures
This Contract shall be signed on behalf of Quick Fitness Repairs LLC by Jose David Nava,
its Managing Partner, and on behalf of Insert Property Tequesta Police Department
By Jeremy Allen, Village Managerits Authorized Representative
Preventative Maintenance Pricing
4 visit per year (Quarterly) at $ 125.00 each starting on: May 22, 2026
Annual Cost $ 500.00
