Fork of Loxahatchee River Dear Mr. Couzzo: As part of the referenced bridge inspection contract, our bridge inspection consultant has notified us that the subject bridge has critical deficiencies that require prompt corrective action (PGA box marked in report). An interim field inspection was performed on 9/23/09 and has noted the following deficiencies (overall NBI ratings for Deck and Superstructure will be lowered to a 3 Serious): As previously noted in a 7/13/09 significant deficiency letter to you, the top and traffic faces of the curbs in Spans 1, 2, 4 and 5 have longitudinal (horizontal) cracks up to 1/16in. wide, some with corrosion staining, and several delaminations, some with corrosion staining. These areas are beginning to spall along the edges of the delaminations. The undersides of the sidewalk slab units have delaminations and spalls, some with exposed reinforcing steel.. The larger delaminated areas in Spans 2 and 4 near mid-span have lost bond with the underlying reinforcing steel. Several of the slab units in Spans 2 and 4 have diagonal cracks up to 12in. long x 1/32in. wide adjacent to the abutment and intermediate bent caps. The undersides of the cast-in-place curb sections in Slab Units 2-3, 2-9, 4-3, 4-9, 5-3 and 5-9, typically have significant cracking up to 1/Sin. wide, delaminations (unsound concrete) and/or spalling as a result of corroded underlying reinforcing steel. In some cases, the delaminations or spalling extend to 3/4 of the span length. The top faces of the sidewalk sections have numerous spalls and delaminations, some with exposed reinforcing steel. The above noted deficiencies still exist. In addition, negative camber exists in Slab Unit 4in Spans 1, 2, 4 and 5. This is reflective in the asphalt surfacing in the deck top over the joints between Slab Units 3 and 4 and 4 and 5 with longitudinal cracking up to 1/bin. wide and minor rutting beginning to occur between Slab Units 1-3 and 1-4. Refer to photo 1. This condition has increased in severity since the inspection dated 6/29/09 when the longitudinal cracks in the asphalt surfacing were first noted. This area is in the right wheel path of westbound lane. The deficiencies in the underside of all slab units showed an increase in severity. Most notable increases were located in Slab Units 2-3 and 2-4. Previously, Slab Unit 2-3 had a 12ft. x aft. delamination. Now, the loose concrete on Slab Unit 2-3 was removed. Slab Unit 2-3 has a 10ft. x 3-1/2ft. x 2-1/tin. deep spalled and delaminated area at mid-span with five transverse reinforcing steel, up to 50% section loss, and six exposed prestressed cables, one having lost bond with concrete and has 100% section loss, approximately 3-1/2ft. west of mid-span. The other five cables have heavy corrosion, but have not lost bond with the concrete and are at mid-span. Refer to photos 2 and 3. Previously, Slab Unit 2-4 had an 8ft. x aft. delaminated patch at mid-span. Now, Slab Unit 2-4 has a 12ft. x aft. X Sin. spalled/delaminated repair area and shows up to 1-1/tin. negative camber at and near mid-span along both edges. In this area, there are two exposed prestressed strands with approx. 20% section loss. One strand has lost bond with the concrete. Refer to photos 4 and 5. Slab Unit 4-4 has tin. of negative camber along the north edge. Refer to photo 6. Negative camber up to 3/4in. was also noted along the north edge of Slab Unit 4-7 eastbound lane. Refer to photo 7. Slab Unit 5-4 has 3/4in. of negative camber along both the north and south edges. This negative camber is occurring at and near mid-span. Refer to photo 8. The deterioration of the slab units and delaminated repairs is on-going and their condition continues to worsen. It is suggested that the slab units be replaced or restored to their original integrity and that the curb and sidewalk sections be repaired. The Village of Tequesta needs to continue to provide immediate actions to correct the noted deficiencies in order to protect the safety of the traveling public (both vehicular and marine) and the Village's investment. www.dot.state.fl.us Sincerely, John Danielsen, P.E.
