As we discussed, ARCADIS and our subconsultant, Bridge Design Associates, Inc., coMa~c:
have incurred additional costs in the performance of work that is considered to be Thomas C. Jensen, PE
outside of our original scope for the above referenced project. This additional work
effort was due to the Village acquiring FDOT stimulus funds for the bridge Plpne:

construction which was not anticipated when this project first commenced. In order to
ensure that this project move forward expeditiously ARCADIS diligently moved err~a:
forward in addressing FDOT issues relative to their internal requirements and Tom.jensen@arcadis-
schedules. As you know, this project was never intended to be a FDOT project, but us.com
once the FDOT stimulus funds were engaged, and FDOT was chosen as the
Administrator (The Village lacked LAP cert~cation), the project quickly turned into a ourret
full FDOT project, with numerous design and drawing format changes, technical RF005106.001
specification revisions, up-front document additions, cost estimating/ schedule of
value re-formatting, several design report submittals, numerous meetings with Fbrida License Numbers
County and FDOT ofFcials, alt of which were completed within compressed
schedules so that the Federal stimulus funding time tables were met. All of this work EBOOOO si9
referenced above was completed between May and August 2009.

There was na time to sit down and determine what the FDOT required, as they
themselves did not know what was expected of them. As they adopted the project as escape Architecture

one of their own, they were constantly having internal issues as to how they could
get this project through to Tallahassee with minimal impact. In the end, they had us Surveying

develop a modified version of a FDOT project which took considerable effort on our
part as you can see from the attached invoice.
The attached invoice is in the amount of $110,523.94 which covers ARCADIS and
Bridge Designs costs between May and August 2009. Please let me know if you

WORK AUTHORIZATION
Tequesta Drive Bridge
"Engineering Services During Construction"
This Work Authorization authorizes ARCADIS G&M, Inc. to perform the work set forth
herein and is issued pursuant to the "Professional Engineering Consulting Services
Agreement for Water System and General Utility/Civil Projects" dated November 17,
2005. All terms and conditions of said agreement shall be applicable to this Work
Authorization.
SCOPE
The Engineer understands that this work authorization shall cover Engineering Services
During Construction (ESDC) as requested by the Village for services required by the
Design Engineer, or for those services not being performed by the on-site CEI or FDOT
representative.
The Engineer has established the team to assist with the ESDC, consisting of the
following consultant;
• Bridge Design Associates, Inc.
The following is a list of tasks that could be performed during the construction
administration of the Tequesta Drive Bridge;
1. Attend the construction status meeting, one meeting per month.
2. Provide responses to RFI's that require input from the Design Engineer.
3. Provide responses to Field requests as required by FDOT and the CEI.
4. Assist FDOT and the CEI on the certifications required by the regulatory agencies.
5. Review and approve shop drawings that require review of the Design Engineer
to include the following;
- Asphalt
- Drainage structures
- Embankment material
- Production piles
- Post -tensioning
- Railings

- Sheet piles
COMPENSATION
For Scope Items 1 thru 5, the Engineer shall be paid according to the fee schedule.
delineated within the November 17, 2005 Agreement on the basis of actual time spent,
plus any applicable taxes and reimbursement for non-salary expenses, including but not
limited to, automobile travel at $0.42/mile, and reproduction.
For services of the other team member that will be sublet, the Engineer shall order their
services after obtaining the written authorization of the Client. The Subcontractor shall
work directly for the Engineer as delineated within the November 17, 2005 agreement.
We estimate the total cost for the services under Scope Item 1 thru 5 will not exceed
Forty -Seven Thousand Dollars and Zero Cents ($47,000.00). This amount shall not be
exceeded without prior written approval by the Client. Refer to the attached fee
breakdown.
TIME OF PERFORMANCE
The Engineer will complete Scope Items 1 thru 5 commensurate with the construction
schedule, which is estimated to be 231 days from November 16, 2009, exclusive of
delays beyond the control of the Engineer.
This authorization can be cancelled with or without cause by the Village with 30 days
written notice. Should said cancellation occur, the Engineer will be paid for all services
performed through the cancellation date.
