A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF
TEQUESTA, PALM BEACH COUNTY, FLORIDA, AWARDING BID TO
CANADA LIFE FOR VILLAGE OF TEQUESTA EMPLOYEES' GROUP LIFE
INSURANCE, AND AUTHORIZING THE VILLAGE MANAGER TO EXECUTE
THE DOCUMENTS NECESSARY TO EFFECTUATE THE SAME ON BEHALF
OF THE VILLAGE.
NOW, THEREFORE BE IT RESOLVED BY THE VILLAGE COUNCIL OF THE VILLAGE
OF TEQUESTA, PALM BEACH COUNTY, FLORIDA AS FOLLOWS:
Section 1. The Proposal from Canada Life for Village of
Tequesta Employees' Group Life Insurance Attached
Hereto as Exhibit "A" and Incorporated by Reference
as Part of this Resolution is Hereby Approved and
the Village Manager of the Village of Tequesta is
Authorized to Execute the Same on Behalf of the
Village, and Other Necessary Documents, Subject to
the Approval of the Proposal by the Communications
Workers of America.
• THE FOREGOING RESOLUTION WAS OFFERED by Councilmember Elizabeth A.
Schauer , who moved its adoption. The motion was seconded
b y Councilmember Basil E. Dalack and upon being put to
a vote, the vote was as follows:

Definition of "Loss" Loss of Use Complete Severance of
with regard to an arm,
leg, hand or foot
As this comparison shows, the Canada Life contract pays a higher percentage for certain
losses than the competition. The major strength of our contract, however, ties in the
definition, of "loss" with regard to an arm, leg, hand or foot. The Canada Life contract
only requires "loss of use" for benefits to be paid. The competition requires "complete
severance of" before any AD &D benefits are paid. This single difference makes the
Canada Life contract infinitely better.
Please note that this is a "general" comparison. We will be happy to provide specific
company examples upon request.

Group Basic Life Insurance
Benefit The full amount of life insurance will be- paid in the event of
an insured employee's death_
Beneficiary An employee, by giving written notice to Canada Life, may
appoint a beneficiary and may change this appointment at
any time.
Waiver of Premium If an insured employee becomes totally disabled prior to age
60, the amount of his/her life insurance may be continued
without payment of premium, Satisfactory initial proof of
disability must be received by Canada Life within twelve
months of the date of disability and proof of continued
disability must be furnished as reasonably required by
Canada Life. The insurance continued under the waiver of
premium provision is subject to any reductions in amount
due to age and will terminate when the employee attains
age 65.
• Conversion Privilege If an insured employee's life insurance is canceled due
to termination of employment or change of class, the
employee may convert the insurance to an individual
policy without submitting evidence of insurability. The
employee's application and the required premium must
be received by Canada Life within 31 days of termination of
employment_
Accelerated Benefit If an employee is diagnosed as having a terminal illness, the
employee may apply to receive 50% of the death benefit up to a
maximmm of $50,000. The employer must consent to the
acceleration and Canada Life must be furnished with
satisfactory proof of the diagnosis. The amount of insurance
payable up on the death of the employee will be reduced by any
amount received by the employee under this provision plus
inerest.

• Accidental Death and Dismemberment
Benefit Accidental Death and Dismemberment Insurance (AD &D)
provides additional protection in the event of loss occurring
within 365 days of an accidental bodily injury.
Schedule of Losses Subject to the limitations below, the following percentage of
the Amount of Insurance in force will be paid in the event of
specified losses.

Limitations l _ In no event will the aggregate amount payable for all losses
resulting #'turn one accident exceed 100% of the Amount of
Insurance,
2. If, as the result of any one accident, an insured person suffers
more than one of the Losses shown in the Schedule of Losses
with respect to any one limb, payment will be made only
for the Loss for which the largest amount is payable.
Exclusions AD &D benefits are not paid for losses resulting from:
1. Intentionally self - inflicted injury.
Z. War, declared or undeclared, or any act of war.
3. Active participation in any riot or violent disorder.
4. The person, either taking or attempting to take their own life
whether they are in possession of their mental faculties or
not at the time_

Accidental Death and Dismemberment
5. Bodily or mental infirmity or illness or disease of any kind,
or medical or surgical treatment Thereof.
6. Poisoning ill any form, inhalation of gas or fumes.
7. Travel or flight in any aircraft except solely as a
passenger in a powered civil aircraft having a valid and
current airworthiness certificate and operated by a duly
licensed or certified pilot while such aircraft is being
used for the sole purpose of tramportation only. Descent
from any aircraft in flight will be deemed to be part of such
flight.
8. Committing or attempting to commit a felony.
9. Bodily injury sustained while operating a motor vehicle
either while under the influence of any intoxicant or if the
blood alcohol concentration at the time of injury is in excess
of the state's legal limitation.
Seat Belt Benefit The employer may elect to include the Seat Belt Benefit with
• the AD &D coverage. In the event of a death resulting from an
automobile accident, while an insured is wearing a seat belt,
Canada Life will pay an additional AD &D benefit equal to the
lesser of:
1. $50,000 or
2. The amount of the AD &D benefit paid because of the
insured's accidental death.
