OF FORT MYERS, FLORIDA, IN THE AMOUNT OF $496,692.00 FOR R.O. WELL NO. 2 CONSTRUCTION AND R.O. WELL NO. 1 DEVELOPMENT/ TESTING, AND AUTHORIZING THE VILLAGE MANAGER TO EXECUTE THE CONTRACT ON BEHALF OF THE VILLAGE. OW, THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF THE VILLAGE OF TEQUESTA, PALM BEACH COUNTY, FLORIDA, AS FOLLOWS: , Section 1. The Bid for R.O. Well No. 2 Construction and R.O. Well No. 1 Development /Testing from Youngquist Brothers, Inc. H of Fort Myers, Florida in the Amount of $496,692.00, Attached Hereto as Exhibit "A" and Incorporated by Reference as Part of this Resolution are Hereby Approved and the Village Manager of the Village of Tequesta is Authorized to Execute the Contract on Behalf of the Village. To hold Bid open until an Agreement has been executed between the Village and the accepted Bidder, or until one hundred twenty (120) calendar days after the bid opening date, whichever is shorter. 3. To enter into and execute an Agreement on the basis of this Bid, within fifteen (15) calendar days after the award of the Contract. To furnish the 100% Performance Bond and 100% Payment Bond in accordance with the Contract Documents. Failure to execute the Contract and furnished said Bonds within fifteen (15) days shall cause bid security to be forfeited, not as penalty, but as fixed and liquidated damages for costs and expenses incurred, payable into the funds of the Village. 4. To start work within ten (10) calendar days of the Date of Commencement as established in the Notice to Proceed. Work • shall be completed within the number of calendar days specified in Article 3 of the Construction Agreement. 96 -141 P -1 5. In the event of multiplication, addition, or clerical errors on the Bid Forms, the Village may correct these errors to clarify the Bids. The unit price called for in this bid includes, drilling a nominal 32 -inch .diameter hole to 325 feet and setting /grouting a 26 -inch diameter steel surface casing to 325 feet, drilling of a pilot hole to 1200 feet, with formation sampling and geophysical logging; ream 26 -inch- diameter hole. to 1200 feet; center, set, and cement 975 feet of 20- inch diameter, 0.375 -inch wall thickness steel casing; drill 500 feet of 20 -inch diameter hole to 1700 feet while collecting flow test data; install temporary wellhead and perform geophysical loggingg; remove temporary wellhead and backfill hole with clean pea size stone to approx. 1450 BLS; center set and grout 1215 feet of 14 -inch PVC pipe and 235 feet of 16 -inch PVC pipe, perform temperature tog on first 3 stages of grouting; develop/test well for 60 hours; conduct 4 -hour step -rate pumping test and 12 -hour constant -rate pumping test (76 hours total); provide 12 hours of standby time; furnish and install permanent well head assembly; provide temporary . tankage with pumps, treatment /monitoring prior to discharge of development /test water; provide 1500 IF of 12 -inch restrained temporary piping and connect to existing reject main; modify reject main diffuser; video of completed well; all in accordance with these specifications.
