SAFETY COMMITTEE
MEETING MINUTES
JANUARY 13, 1997
I. CALL TO ORDER AND ROLL CALL
The Tequesta Safety Committee held its regular meeting at
the Village Hall, 357 Tequesta Drive, Tequesta, Florida, on
Monday, January 13, 1997. The meeting was called to order
at 2:05 P.M. by Chairman Tom Hall, Water System Manager.
Committeemembers present were: William Allen, Code
Enforcement Officer Dave Davis, Archie Garrett, Joanne
McCabe, Public Works Director Gary Preston, and Police Chief
Stephen Allison. Fire Chief James Weinand arrived at 2:07
P.M. Absent from the meeting were Bob Pieris and Village
Clerk Joann Manganiello.
II. APPROVAL OF AGENDA
Gary Preston made a motion to accept the agenda as
submitted. Motion was seconded by Joanne McCabe. The vote
on the motion was:

IV. OLD HUSINESS
A) Review �hang�� in Draft of Gener�l Safety Rules
Chairman Hall repor�ed t�hat the major change in the
general Village-wide sa�ety rules wa� in Item 5,
regarding employees under th:e influence of drugs or.
alcohoJ. Qn the job, which had previously indicated anly
ane option, tY�at of imrned:iate dismissal. A change was
made to state that those employees "may be subject to
dsscip.t�ne, up td and inc.luding termination, .�mployees
tak�ng pre�cribed medication s�.ould adv�se their

Tequesta Safety Ru1es would be forwarded to the Village
Manage� and to the Village Labor At�orney for review and
to union representatives for their comments. Mr. Hall
explained that thes� were general safety rules which all
employees should use as a guideline to govern themselves,
a�d that each department had additional site specific
safety rules.
Gary Preston made a motion to aceep� the revised Village
of Tequesta Safety Rules and to forward them to the
Yillage Manager's office for further review. Motion was
seconded by Joanne McCabe. The vote on the motion was:

IV. NEW BUSINESS
A) Presentation by Ms. Julie 2iaja, Senior Loss Prevention
Speciaiist, RISCORP - Accident Investigation
Ms. Ziaja distributed information. to the Committeemembers
regarding claims management, which she commented would
cover many of the claims questions she had been asked at
the last meeting. Ms. Ziaja explained that this
information could be used as a handy reference for many
items, including how the amount of wages was determined,
indepen.dent contractar questions, comin.g and going to
work rules, recreation and social activities, on-the-job
traveling, etc. One of the items covered was
subrogation., which was related to the video to be shown
regarding accident investigation, since it might be
learned that it was possible to obtain payment from the

carrier of another party responsible in some way for the
accident. An example given by Ms. Ziaja was product
liability in which a piece of equipment used that caused
an accident was found to be faulty.
A video titled How to Investigate an Accident cvas shown.
Benefits of investigating every accident, regardless of
size or severity, were covered. Procedures for proper
�investigation were reviewed, and accident pre�ention was
stressed as an integral part of any safety program. Ms.
Ziaja recapped the information presented in the video:
(1} Priznary reasons for investigating an accident were
to establish the facts for future reference and
records, to determine whether a pattern was in
place such as accidents happening at certain times
of day, ta correct physicaZ hazards, and to
determine what training was needed by employees;
(2} Supervisors would investigate and fill out the
initial report;
(3) Th� Committee as a whole would only analyze the
data except in the case of a major accident when a
member of the Safety Committee might be needed to
help investigate;
(4) The accident scene must be preserved, and
investigation must be dane timely, with notes made
from in.terviews with the injured party or any
witnesses, and the area photographed or videotaped
in some instances.
(5) Judgements should not be made and general
statements avoided, i.e., details should be the
focus of the investigation.
(6) Whether any outside party contributed to the
accident should be investigated immediately.
(7) After analyzing the data, similar areas where the
same type of incident might happen should be
identified.

(8) Any evidence, including photographs of the scene,
records of witnesses, accident reports, and any
tools or equipment used should be preserved.
In response to a question by Police Lt. Gar1o, Ms. Ziaja
advised that a notice of injury claim form should be
submitted when medical treatment was obtained, or if it
was a muscular injury or any type of injury that might
produce delayed symptoms. If an employee refused medical
attention, and symptoms appeared later, there was seven
days to file the report.
Ms. Ziaja provided posters on preventing slip and fall
injuries and proper lifting, and advised that she co�ld
obtain other copies if different departments desired
them.
Chairman Hall commented that from what he had noted in
the video that the Supervisors Investigation Report
provided by RISCORP appeared to need additional
information. Ms. Ziaja explained that the form was a
general form provided as a guide and that it cauld be
catered to fit specific needs.
B) Review Inspection Form Sampie
Chairman Hall requested the Departmen.t Heads on the
Safety Committee review the inspection farm sample, and
make suggestions for additional items. Chairman Hall
commented that a master file would be kept containing a
report of each accident, which the Safety Committee would
review and possibly make recommendations to the Village
Council. Chief Weinand advised that his department had
State forms and that Village-wide notice af injury forms
were needed. Chairman Hall explained that this form was
a follow-up to the injury report. Chief Allison
commented that the Police Department did not have a
follow-up form.
Chairman Hall an.nounced that at the next meeting the form
wauld be reviewed and changes made to produce a universal
investigative form to be filled out by supervisors, kept

in. a master file, and reviewed by the Committee.
Chairman. Hall explained that a recommendation form for
submittal to the Village Manager would also need to be
developed. Chiet Weinand suggested an amendment to the
form the supervisor would fill out so that he would know
someon.e else would also be conducting a review and would
therefore fill out the form more carefully. Chairman
Hall reques'ted that Chief Weinand bring a written
amendment to the next meeting.
VI. ANY OTHER MATTERS
A) Suggested Committee Input Regarding Agenda Items
Chairman Hall requested that Committeemembers submit
suggestions to hirn for agenda items.
Chairman Hall commented tha� as soon as the follow-up
form was completed tha� a plan should be made to conduct
inspections, possibly a monthly Village-wide inspection
by two Gommitteernembers (one Department Head and one
regular employee). Chief Weinand expressed his opinion
that policies and procedures should be put into place
before conducting inspections. Mr. Hall comm�nted that
the first inspections would only be cursory, but could
p�ck up obvious safety hazards which needed to be
addressed, since people tended to overlook things in
their own work area that an outsider might notice.
Chairman Hall reported that the Drug Free Workplace
policy would soon be forwarded an to various departments
and bargaining units. General Safety Ru1.es should be
adopted within the next month. Chairman Hall requested
help from the other Committeemembers.
B) Set Date and Time for Next Meeting
Date and time of the next meeting was set for Monday, 27,
1997 at 2 P.M.
