Please find attached a Purchase Order Requisition to add to the Village Council agenda. This
purchase order would allow Altneida Industries of Fort Lauderdale, Fl to repair and replace
the roof damage of the Filter Plant Control Building at the Tequesta Water Plant. This
damage was inflicted upon the Filter Plant Control Building as a result of Hurricane Frances.
This contract amount will be reimbursed by insurance monies.
Please place this item on the Apri114, 2005 Village Council agenda for consideraaor�.

VILLAGE OF TEQUESTA, PALM BEACH COUNTY, FLORIDA,
AWARDING A CONTRACT TO ALMEIDA INDUSTRIES, INC
OF FORT LAUDERDALE, FLORIDA, IN THE AMOUNT OF
$21,750.00, FOR THE REPAIR OF THE ROOF 4F BUILDING #4
AT TEQUESTA WATER TREATMENT PLANT AND
AUTHORIZiNG THE VILLAGE MANAGER TO EXECUTE THE
APPLICABLE CONTRACT ON BEHALF OF THE VILLAGE.
WHEREAS, the Village of Tequesta has received
insurance reimbursement proceeds to pay for
this repair,
WHEREAS, the Village of Tequesta has budgeted
this repair into account #401-242-546.303,
NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE
COUNCIL OF THE VILLAGE OF TEQUESTA, PALM BEACH
COUNTY, FLORIDA, AS FOLLOWS:
Section 1. Consideration to award a bid proposal to Almeida Industries,
Inc., of Fort Lauderdale, Florida, in the amount of $21,750.00, for the repair
of the roof of building #4 at Tequesta. Water Treatment Plant attached
proposal hereto as E�ibit "A" and incorporated by reference as a part of
this Resolution is hereby approved and the Village Manager of the Village
of Tequesta. is authorized to execute the same on behalf of the Village.
THE FOREGOING RESOLUTION WAS OFFERED by Councilmember

We propose to furnish all materials, labor, and equipment for the re-rooCng on the above referenced project
per the foltowing scope of work, as determined by the results ot our recent on site inspection:
° Proposed Roof Area
Flat Roof Surface approximately 2,720 square feet
Scope of Work Flat — Stanciard BUR RoofSystem
1. Have pre-job meeting with AI Roofing and Owner to discuss job set-up, safety, staging,
and construction time line.
2. Have a pre-construction interior inspection of the proposed area with a representative
of the building to identify any existing problem areas.
3. Tear off the damaged roof surfaces down to the existing concrete decking and dispose
of. (Trash cans provided by AI Roofing and left on-site during roof process)
4. Furnish and install new wood nailer to al! open perimeters with approved fasteners per
locat building codes.
5. Furnish and install a quick dry primer to the entire concrete decking.
6. Furnish and install new pressure treated 1" x 4" wood nailer to the entire perimeter
secured using approved fasteners per local building code requirements.
7. Furnish and install new wood fiber cant strip material to alt curb junction areas.
8. Furnish and install new CERTIANTED GLASBASE #75 fibergtass base sheet (or
equivalent) per manufacturer's installation instructions securing it to the decking with
approved fasteners per local building code requirements.

base sheet in full moping of hot asphalt.
� 10. Furnish and install two (2) plies of CERTIANTEED FLINTGLAS Ply IV fiberglass
reinforced roo�ng felts (or equivatent) over the newly install insulation �board in fuil
moping of hot asphalt.

project completion and receipt of final payment.

above specifications, for the sum of
Scope of Work Flat - Standard BUR Roof System:
Twenty One Thousand Seven Hundred Fifty Dollars ($21,750.00)
Payments to be made as follows:
20% ($4,350.00) upon acceptance of proposal, 40% ($8,700.00) upon delivery of materials,
and remaining 40% {$8,700.00) halance due immediately upon completion.
All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to standard practices. Contractor
retains [he initial right to romedy any consequential damages at the sole discretion of contractor and shall not be held liable for any damages
occurting previous to or fo�lowing performance of contracted work. Any alteration or deviation from specifications involving extra costs, will
be executed only upon written orders, and will become an extra charge over and above the estimate. All agteements contingent upon strikes,
accidents or delays beyond our control. Owner to carry fire, rornado, and other necessary insurance. Time is of the essence conceming this
valuable contract. This project may be stopped if progress payments are not made on time. (f it becomes necessary for this contractor to expend
legal fees to collect our moneys these legal fees wiU be the responsibility of the property owner. Signer can be personally liable. l.5% interest per
month after lU days. All warranties to be issued upon completion o contract.
