Asdiscussed, we visited the WTP|ast week on matters related to operations
but noted the condition of the referenced tank. These are the same tanks we had
previously recommended for replacement and were planned for incorporation into a
construction package to include other important rehabilitation items. We noted that
the north tank is leaning slightly. It is not clear whether this as a new development or
just previously unnoticed and is also not clear whether it indicates imminent failure,
or not. However, it is of significant concern that we feel the effort to replace the tanks
should be pursued with a higher degree of urgency. We recommend that
design/permit efforts should commence at the earliest possible date. We also
recommend that his be treated as an emergency to avoid the delays inherent with
conventional bidding. In |ieu, thereof, we recommend that quotes to oeouna and
install the bank be obtained from two or three properly licensed Contractors with
whom the Vi|leBo, and we, have previous reasonable experience and the project
awarded bzthe lowest price company.
As we diaouesed, a revised work authorization to cover associated
emergency services is included heren, should you agree with this approach and
elect to proceed accordingly. Obviously, we will also modify and forward the previous
work authorization toreflect the removal ofthese tanks from the prior, larger project.

WORK AUTHORIZATION
YTP — Hypochlorite Tank Replacement
This Work Authorization authorizes ARCADIS G&M, Inc. to perform the work set forth herein
and is issued pursuant to the "Professional Engineering Consulting Services Agreement for Water
System and General Utility/Civil Projects", dated November 17, 2005. All terms and conditions
of said agreement shall be applicable to this Work Authorization.
SCOPE
This project includes design, permitting and engineering construction services for the replacement
of the sodium hypochlorite tanks at the water treatment plant.
The Engineer will perform the following tasks.
1. Prepare finals plans, specifications and bidding documents for the work and review with
the owner.
2. Prepare PBCHD permit documents, submit the permit application and act as the owner's
liaison with the agency during the permitting process.
3. Prepare a construction cost estimate.
4. Assist the owner with securing a quotation from two or three qualified Contractors.
Evaluate the quotes and make a recommendation of award. Note, the project will not be
conventionally bid.
5. Perform all `Engineering' duties as set forth in the construction contract and administer
the construction contract.
6. Provide part-time inspection services during construction.
7. Provide periodic site visits from the Engineer during construction.
8. Certify project completion to the PB CHIT upon completion.

COMPENSATION
For Scope Items 1 thru 8, the Engineer shall be paid a lump sum fee of twelve thousand dollars
($12,000). The Engineer has assumed all permitting fees will be paid directly to the agencies by
the Owner.
TIME of PERFORMANCE
The Engineer will complete Scope Items I thru 3 within 30 days of the receipt of an executed
authorization, exclusive of delays beyond the control of the Engineer.
This authorization can be cancelled with or without cause by the Village with 30 days written
notice. Should said cancellation occur, the Engineer will be paid for all services performed
through the cancellation date.
