COUZZO, VILLAGE MANAGER FROM: MATT HALUCK, PUBLIC WORKS SUPERVISOR SUBJECT: VILLAGE HALL AIR CONDITIONING CONTINUING MAINTENANCE DATE: JULY 11, 2013 Per your request, I have solicited a continuing contract for service on the Village Hall Air Conditioning system. CMI is providing the repairs and I feel they are very capable, familiar, and competent to maintain the current system. The agreement will be for one (1) year on a quarterly basis. Also, if you agree to the service agreement I will need to amend the current budget to incorporate the additional costs totaling $2256.00, (account 546.303 from $8000 to $10,256.00) which will include tune -ups and filter changes outlined on their proposal. Attached is the agreement form along with my suggested quarterly months for service. July 15, 2013 Air Conditioning Continuing Maintenance Agreement ADDENDUM Public Records: In accordance with Sec. 119.0701, Florida Statutes, the Contractor must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Scope of Services. Upon request, the Contractor must provide the public with access to such records in accordance with access and cost requirements of Chapter 119, Florida Statutes. Further, the Contractor shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Scope of Services are not disclosed except as authorized by law. Finally, the Contractor shall retain the records described in this paragraph throughout the performance of the work described in the Scope of Services, and at the conclusion of said work, transfer to the Town, at no cost to the Town, all such records in the possession of the Contractor and destroy any duplicates thereof. Records that are stored electronically must be transferred to the Town in a format that is compatible with the Town's information technology systems.
