Causley, Inc. is pleased to submit the following Proposal to provide the Village of Tequesta with Building Department Services. The scope of services includes, but is not limited to, fulfilling the duties of Chief Building Official, Plan Examiners, Inspectors, as well as permit technicians and shall include the areas of building, building roofing, plumbing, plumbing gas, electric, mechanical, and HVAC, as well as emergency and disaster response services. RESOURCES M.T. Causley's workforce consists of Building Officials, Inspectors, Plans Examiners, Code Enforcement Officers, administrative and permitting staff boasting many years of successful experience. The professional team is licensed by the State of Florida and together hold over 30 different types of licenses. Even though the main office is located in Homestead, M.T. Causley employs staff members which reside throughout Florida. Not only does this allow M.T. Causley the opportunity to assist cities and counties throughout the State, it also provides coverage in the event of natural disasters. Our clients rely on M.T. CAUSLEY, INC BUILDING & GOVERNMENT DEPARTMENT SERVICES Scope of Services Page 5 Approach M.T. CAUSLEY, INC. BUILDING & GOVERNMENT DEPARTMENT SERVICES M.T. Causley understands that the Village of Tequesta desires to secure a qualified firm with the experience and technical qualifications to provide Building Department Services. To accomplish this successfully, M.T. Causley will work diligently to ensure the needs of the Village are met, providing services in a timely fashion and within budget, while optimizing the level of service to the Town. M.T. Causley proposes to supply a primary and alternate team of multi - certified Plans Examiners and Inspectors, as well as Building Officials who are properly licensed and certified in accordance with F.S. 455, 468, 471, 481, and 489. Staff will perform plan review and inspections services during the Village's designated hours in accordance with the Florida Building Code, Village amendments, and applicable codes. Staff will be provided with the necessary equipment to fulfill their duties. In addition to Building Department Services, staff will perform any other related services required by the Village and all other governmental agencies having jurisdiction. The following provides a detailed approach and methodology to providing services to the Town. Building Official The designated Building Official will oversee compliance with the Villages's adopted standard code, Florida Building Code, National Electrical Code, Unsafe Building Abatement Code, and FEMA Regulations. Additionally, the Building Official will administrate all plan review, inspections and enforcement. Services will be coordinated with Village's Planning and Zoning Consultants, Village's Fire Marshall, and Villages Code Enforcement Officers. Review of Plans • Staff will review plans at the Village's Building Department Division during normal business hours. Review construction plans for new single and multi - family dwellings, new non - residential development and all residential and commercial additions /re- models for compliance with codes. BUILDING & GOVERNMENT DEPARTMENT SERVICES • Plan review shall include, but not be limited to building, roofing, mechanical, HVAC, plumbing, structural and electrical. • Review of plans will result in recommendation of approval, approval with modifications, or denial of the plans with clear and specific written findings and observations that support the recommendations. Reviewers will upload information utilizing the Village's permitting system. • Walk -thru plans review services will be provided during Village designated times for minor permits. Staff will be available to review comments with architects, contractors, developers, and property owners to obtain necessary changes for approval. Staff will determine building permit valuations for construction projects, calculate impact fees. Provide the Village with recommended code revisions that are necessary or desired. M.T. Causley will review and maintain all records required by FEMA in association with the processing of building permits in the format required by FEMA and Flood Plain Manager. Inspections All inspections shall be performed within 24 hours of request during normal business hours, Monday thru Friday, excluding U.S. Federal Holidays and natural disasters. Staff will inspect permitted construction within the Village limits for compliance with FBC, NEC, Village codes and ordinances and permitted plans and specifications. Inspectors will advise and confer with architects, engineers and Village personnel, as well as homeowners, contractors and developers. Questions, concerns and inquires shall be received and responded to within one (1) business day. Page 7 Approach M.T. CAUSLEY, INC. BUILDING & GOVERNMENT DEPARTMENT SERVICES Inspectors will conduct technical field inspections of new, repair, remodel, demolition and alteration of industrial, residential and commercial buildings during the various phases of construction, installation and operation, assuring work conforms to all applicable building codes and approved plans and specifications. Written comments will be issued if the work is not in accordance with the approved plans or applicable codes. Inspectors will maintain formal inspection records provided by the Village. Inspection results will be entered in the Village's permit system. Staff will conduct change of occupancy inspections as well as investigate complaints, hazardous building inspections and assist with prosecution of building code and hazardous building violations. Staff will provide written reports of complaints, provide adequate alternatives to bring buildings into compliance and clarify the intent of codes and ordinances. • Staff will provide services with regard to Unsafe Buildings, inspect, post, and record violations and conduct public hearings and provide support to the Unsafe Structures Board. Record Keepin_g All plans, sketches, drawings, computation details, inspection reports and other documents will be the property of the Village. M.T. Causley will work with the Village to ensure proper storage of files. Emergency Services Staff will be available to respond to any type of emergency requested by the Village's Law Enforcement or the Village's authorized representative within one hour. Mr. Causley will be the primary point -of- contact and can be reached via cell phone at any time. In the event of a declared emergency, M.T. Causley will staff the EOC and will assist with damage assessment and safety inspections. M.T. Causley will work with the Village during post disaster times in restoring Plan Review and Inspection Services. Page 8 Approach M.T. CAUSLEY, INC. BUILDING R GOVERNMENT DEPARTMENT SERVICES Damage Assessment M.T. Causley has extensive experience in providing damage assessment services. M.T. Causley has first -hand knowledge of what needs to be done in the event of a disaster and the experience to accomplish those tasks in a successful and efficient manner. In addition to staff experienced in disaster assessment and reconstruction, M.T. Causley's staff includes Flood Plain Managers. A FEMA Coordinator is also available if needed. Extended hours may be available to meet the needs of the Village and the community prior to and following a natural disaster. Many of our staff are trained in damage assessment. If the Village were to experience an unfortunate disaster, local staff would obviously be consumed with their normal duties, as well as personal concerns. The benefit of M.T. Causley having employees located throughout the state including Broward, Miami -Dade, Palm Beach, Monroe, Marion and Pasco Counties, as well as Tennessee, is that these team members would be available to provide necessary relief and added services to the Village. Customer Service Requirements M.T. Causley provides customer service standards for all personnel and will also follow the Village's guidelines. Staff shall greet each customer presenting to the office and by phone in a courteous and professional manner. Questions, concerns, and phone calls shall be returned within one business day. Flexible work schedules may be established to extend office hours and services following a natural disaster to perform special code compliance sweeps and enforce building code regulations and other unexpected issues. M.T. Causley's staff are well -known for their customer service skills throughout Florida. We strive to provide a professional, welcoming atmosphere in the building department. It is our goal to work with customers to reach optimum outcomes. Our staff will blend in well with Page 9 Approach M.T. CAUSLEY, INC. BUILDING & GOVERNMENT DEPARTMENT SERVICES the Village staff to create a cohesive working relationship. M.T. Causley is able to assist the Village staff with implementing additional customer service standards which include scripting for telephone and counter etiquette, as well as appearance and non - verbal issues if requested. M.T. Causley can assure the Village that customer service is of extreme importance as our reputation depends on it. It is our goal to assist contractors, developers and property owners in a courteous and professional manner. Rather than saying "No" to a customer, we believe it is important to try to find a way to assist them in achieving their goal and yet ensure the Florida Building Code and all related codes are complied with. Identification Staff shall be identified with a Village of Tequesta identification badge, visible and displayed at all times. Staff will wear polo shirts identifying their name and the M.T. Causley logo. M.T. Causley's vehicles are identified with M.T. Causley's logo and are well maintained, clean and in safe operating conditions. Equipment Field staff are equipped with company vehicles, cellular phones, laptops, cameras, and the equipment necessary to fulfill their duties. Field staff are required to possess and maintain a valid Florida driver's license. As a result of the transportation and technology available to our staff, the need to physically "check -in" at our offices is eliminated. Our staff becomes your staff and the fact that they are contract employees ^ is transparent to customers. Administrative Services: vi Technical staff will assist Village staff and applicants in interpretation of adopted local and state codes as required. Permit packages will be reviewed for completeness, accuracy and compliance with the Florida Building Code upon submittal. Page 10 Approach M.T. CAUSLEY, INC. BUILDING & GOVERNMENT DEPARTMENT SERVICES Upon statutory code revisions, staff will coordinate with Village staff in updating permit application forms and submittal guidelines as needed. Furthermore, amendments to the Florida Building Code will be reviewed and updated upon adoption of new Florida Building Code. Updates of forms and information relating to the Building Department with the Village's website administrator. Additional Services • Provide zoning compliance (building permits, business tax receipts, occupational licenses). Work with facility management on all Village bids for construction or maintenance, repairs, etc. Services to be assigned to third party, when required: Annexations Land use Amendments Rezonings Special Exceptions Site & Developments Plans Variances Facilities The Village will provide personnel with facilities and support services adequate for the performance of their duties, including office space, computers, office materials and equipment, including access to Village clerical support. Causley, Inc. is committed to providing a safe, efficient, and productive environment for all employees. Therefore, job applicants and current employees are asked to provide body substance samples (such as urine and /or blood) to determine the possibility of illegal use of drugs or alcohol. Applicants and current employees working in proximity of children will have a Level 2 background check. Any applicant who fails the drug test or the Level 2 background check will not be accepted for employment. Any employee who refuses to submit to drug testing is subject to disciplinary action up to and including termination of employment. Employees are required to read the Drug -Free Workplace Policy and sign a statement acknowledging their understanding of the policy and intent to follow the policy. Additionally, employees must sign the Background Check and Credit History authorization included in the Employee Handbook (sample has been included in the following page). Background checks and drug test screenings are conducted by InfoChecks USA, LLC. Included in the background checks, M.T. Causley requests a complete history from the Department of Motor Vehicles. Additionally, M.T. Causley leases its vehicles through Enterprise Rental Car. Enterprise supplies M.T. Causley with monthly reports detailing the activity of each vehicle, including the speed that vehicles are driven and traffic violations. Fee Structure, Compensation and Billing M.T. Causley will work with the Village in establishing services the Village may need to ensure the building department is working as efficiently and customer - service oriented as possible. M.T. Causley will provide a monthly statement for services rendered, and will track items including, but not exclusive to, number of permits issued, total hours for plan review by trade, inspectors hours, number of inspections by trade, time per inspection in total and time to perform actual inspection. Page 12 BACKGROUND CHECK AND CREDIT HISTORY AUTHORIZATION & ORDER FORM Under the applicable provisions of the federal Fair Credit Reporting Ad (FCRA), notice is hereby given that a consumer report of investigative consumer report may be made which may include information pertaining to your employment history, educational accompli shi rents, criminal record, driving record, credit history, charader, general reputation and personal characteristics. Causley proposes to supply the Village of Tequesta Building Department Service based on the following hourly rates: STANDARD BEYOND 5 PM SERVICE HOURLY RATE SATURDAYS* Building Official $75 per hour $112.50 per hour Multi- certified Plans Examiner & Inspector $70 per hour $105.00 per hour Single Trade Plans Examiners: $68 per hour $102.00 per hour (Building, Mechanical, Electrical & Plumbing) Single Trade Inspectors: $68 per hour $102.00 per hour (Electrical, Mechanical, Plumbing and Building) In the event of a natural disaster, the City will be $65 per hour, plus $97.50 per hour, provided with qualified staff to conduct damage expenses i.e., plus expenses i.e., assessment services lodging, travel and lodging, travel, and meals meals Services requested for US Federal recognized holidays and Sundays will be provided at 2 times the standard hourly rate. Page 15 Public Records: In accordance with Sec. 119.0701, Florida Statutes, the Contractor must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Scope of Services. Upon request, the Contractor must provide the public with access to such records in accordance with access and cost requirements of Chapter 119, Florida Statutes. Further, the Contractor shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Scope of Services are not disclosed except as authorized by law. Finally, the Contractor shall retain the records described in this paragraph throughout the performance of the work described in the Scope of Services, and at the conclusion of said work, transfer to the Village, at no cost to the Village, all such records in the possession of the Contractor and destroy any duplicates thereof. Records that are stored electronically must be transferred to the Village in a format that is compatible with the Village's information technology systems.
