Due to the exclusion prxess, there may be situations when rodents are sealed out and not trapped on the inside. p� $ 350.00 � RODENT MONITORING AGREEMENT This program continues the rodent prooflng guarantee when rodent proofing has been purchased. The initial charge is for the installa- tion and batting of the Rodent Monitoring Stations and is based on the number of stations installed. The servfce agreement is Monthiy or Every Other Month based on Rodent Pressure and includes rebaiting/refreshing bait and cleaning interior of statlons. Initial Price $ 350.00 Services X 11 Total Price $ 680.00 RODENT PROOFlNG SPECIAL SERVICE This is for the exclusion ofi the Norway Rat, Roof Rat and Hause Mouse. We will make minor repairs and install coverings and shields to deny axess to the structure from the exterior. This service is guaranteed for 30 days in the absence of a monitoring agreement. YOUR COOPERATION: Your cooperation is important to ensure the most effective results from Nozzle Nolen, Inc.. In the event you have to reschedule the service, please notify Noule Nolen, lnc. within 24 hours in advance. ACCESSIBILITY: Please make sure pets/animals are secured and accesses/gates are unlocked for service. If for any • reason the pets are not secured and/or the gates are locked, prohibiting access and preventing service from being • performed, the service amount will be due from the customer. We will not be obligated to make up or add an additional service at the property. DAMAGES: This ,4greement does not guarantee against, and Nozzle Nolen, Inc. shall not be liable for, any consequential damages of whatever nature, including damages to persons, animals, or property. In many cases rodent exclusion may require working on the roof. Nozzle Nolen will not be held responsible for damage caused such as cracked, broken tiles or shingles. � TERMS OF SALE/BUYER'S RIGHT TO CANCEL: In consideration of the above service packages, the undersigned agrees to pay in a timely manner. The initial/first payment is due at signing of this Agreement. Service may have tax included, depending on local state law. Terms of sale are met at the time of service. A 1.5% per month, (18.00% annual) service charge will be imposed for delinquent accounts. Customer may cancel this transaction (in writing) at any time prior to midnight the third business day after the date of this transaction. The Customer acknowledges that he/she has read and understood this Agreement and will abide by its terms. The "Auto Debit° payment plan authorizes Nozzle Nolen, Inc. to automatically withdraw the monies due after each service is rendered. Checks returned for any reason will be charged a$25 reprocessing fee. Necessary price adjustments may be negotiated anytime after the first year. NOZZLE NOLEN, INC. OWNERSHIP OF EQUIPMENT: The Customer understands that ali components used for the elimination and/or prevention of the covered pests will remain the property of Nozzle Nolen, Inc., unless otherwise purchased separately by the Customer. The Customer has no rights to any of the components on the Customer's premises under this Agreement, other than the right to use as instailed by Nozzle Nolen, Inc. On the expiration or or termination of this agreement, Nozzle Nolen, Inc. is authorized by the Customer to retrieve the components from the Customer's premises. This service and its components are fully transferable to the new owner should a sale of the premises necessitate. ASSIGNABILITY: Nozzle Nolen, Inc. resenres the right to perform services through an independent contractor it selects. ADDITIONS, ALTERATIONS AND OTHER CHANGES: This Agreement covers the Structure(s) identified on the service agreement as of the date of the initial service. The Customer will immediately notify Nozzle Nolen, Inc. in writing of any changes to the square footage of the Structure. Failure to notify Nozzle Nolen, Inc. in writing may void this Agreement. Any changes to the square footage of the Structure may warrant an adjustment in the annual fee to guarantee a pest-free environment. HOLIDAYS: if your service date falls on a holiday, in most cases, your service will be done the business day prior or after the holiday. RENEWAL OF AGREEMENT: Rodent Monitoring Agreement will automatically renew each year (subject to price increase). CANCELLATION OF AGREEMENT: It is understood that should the Rodent Monitoring Agreement be cancelled, for any reason, before the first year of service is completed, above account will pay a cancellation fee at the charge of one regular service visit. Nozzle Nolen, Inc. and Owner agree that as to any civil action or legal proceedings arising out of or relating to the Rodent Monitoring Agreement, the laws of the State of Florida apply and venue shall be Palm Beach County. � �There rnay be situat�ons when rodents are gainrng access to the structure from the ansule. They are usually non-vrsible such as broken or abandoned sanitary or conduit lines. We may recommend that a pCumber or electrical contractor be eosed to perd'orm tests such as a smoke or inlane camera test and to seal sep any access points for�raL These services are zaot included a�nder ihe scope of th�s agreemen� ADDENDUM Public Records: In accordance with Sec. 119.0701, Florida Statutes, the Contractor must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Scope of Services. Upon request, the Contractor must provide the public with access to such records in accordance with access and cost requirements of Chapter 119, Florida Statutes. Further, the Contractor shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Scope of Services are not disclosed except as authorized by law. Finally, the Contractor shall retain the records described in this paragraph throughout the performance of the work described in the Scope of Services, and at the conclusion of said work, transfer to the Village, at no cost to the Village, all such records in the possession of the Contractor and destroy any duplicates thereof. Records that are stored electronically must be transferred to the Village in a format that is compatible with the Village's information technology systems.
