A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF
TEQUESTA, FLORIDA, AWARDING A ONE YEAR CONTRACT TO
SHEEHAN TURF CARE INC. OF JUPITER, FL FOR THE
MAINTENANCE OF THE BALLFIELDS AT TEQUESTA PARK IN THE
AMOUNT OF $30,000.00, PROVIDING FOR SEVERABILITY;
PROVIDING AN EFFECTIVE DATE; AND FOR OTHER PURPOSES.
WHEREAS, the Village of Tequesta seeks to enter into a one {1) year contract with
Sheehan Turf Care, of Jupiter, FL to maintain the ballfields at Tequesta Park.
WHEREAS, the Public Works Department will have funding available in its Landscape
and Irrigation Maintenance account in the Fiscal Year 2015/2016.
NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF THE
VILLAGE OF TEQUESTA, PALM BEAC�i COUNTY, FLORIDA, AS FOLLOWS:
Section 1 Resolution No. is hereby approved awarding a one (1) year contract to
Sheehan Turf Care, Inc. of Jupiter, FL in the amount of $30,000.00 for the maintenance
of the ballfields of Tequesta Park.
Sectioa 2 This Resolution shall become effective immediately upon passage.

Proposal
This service agreement between Village Of Tequesta /ocated at Tequesta Park,
Tequesta, FL 33469 and Sheehan Turf Care, lnc., an independent contractor
will be valid October , 2015 thru Sepfember , 2016
1. SERV/CES TO BE RENDERED AS PART OF THE MONTHLY SERVICE:
A. MOW/NG: Weekly mowing of fields, frequency is determined by seasonal
variations. Minimum 52 cuts.
B. EDGING: Waming track areas will be chemically treated once a month.
C. FERTILIZATION: Sheehan Turf Care will fertilize all turf as follows.
a. Monthly checks on all irrigation on ball fie/ds. Heads will be installed
at the price of $32.00 per Head.
b. Spring and fal! application of Dylox for control of grubs and mo/e
crickets.
c. Every 3 months application of 19-0-19 Pre-M (Weed control)
d. Applications will be made quarterly 10-4-20
D. PEST CONTROL: Sheehan Turf Care will perform weekly checks for pest
in al/ turf areas with control methods to follow if present.
The services in the service agreement to be provided for a Monthly fee of
$2500.00
ADDITIONAL SERVICES
If requested, the following services will be performed and billed on a"time and
materials" basis:

THE TOTAL COST FOR THE SERVICES MENTIONED ABOVE ARE $ 4320.00 /MONTH
Billing/Payment: Services will be billed on the 1 of every month, and payment will be due by the 15 of the
following month. Please Remit to Vivid Green P.O. Box 4737 W.P.B. FL. 33402.
(Any party can cancel with 30 day written notice at any time or if contract is not being fulfilled.)
*Trash will be removed as needed throughout the property except excessive debris caused by uncontrollable circumstances
(natural or human destruction). At which time a proposal can be provided for cleanup.
*Enhancements: All removals planting and mulching will be under a separate contract labor rate is $30 to $35 per man hour
plus material costs.
*Liability: Contractor accepts all liability of its work, workers and equipment while on property. An English speaking supervisor
will be on property while work is being performed.

INTO A CONTACTUAL RELATIONSHIP FOR MAINTENANCE OF LAWN AND LANDSCAPE SERVICES,

CUTTING ALL SODDED AREAS (52� TIMES ON AN ANNUAL BASIS.

2) TRIMMING WITH LINE TRIMMER AROUND ALL STRUCTURES NOT REACHABLE TO LAWN
MOWERS
3) CLEANING OF ALL WALKWAYS, DRIVEWAYS AND STREETS (SZ) TIMES PER ANNUALLY
4) PICK UP AND DISPOSE OF ALL PAPERS, CANS, BOTTLES OR ANY OTHER - fYPE OF FOREIGN
OBJECT UPON EACH SERVICE. HOWEVER, PLEASE TAKE NOTE THAT IN BETWEEN OUR SERVICE TO
YOUR ESTABLISHMENT WE WOULD ASK THAT YOUR PERSONNEL/RESIDENTS CONTRIBUTE TO
KEEPING THE PROPERTY CLEAN OF GARBAGE IN THE ABILITY AS BEST AS YOU CAN.

3) IN CONJUNCTION WITH THE TRIM SERVICE, OR IF REQUIRED "AS NEEDED" fiLONG W�TH
TURF SERVICE, ALL PALMS & DESIDUOUS TREES WILL BE MAINTAINTED IN A WAY
(HARDWOOD TREES�UP TO 8'� SUCH THAT THEIR LIMBS WILL NOT POSE AN
OBSTRUCTION TO PEDESTIANS OR BE A POTENTIAL LIABIUTY. AS IN THE CASE OF PALMS,
THE TECH MAY REACH AT LEVEL GROUND WITH AN EXTENDED POLE SAW (PALMS �UP
TO 14"� AND REMOVE LOW HANGING, AS WELL AS DEAD LIMBS.
4) THIS IS IN NO WAY TO BE CONSTRUED AS A COMPREHENSIVE TREE TRIMMING. THIS
SERVICE WILL BE OFFERED UPON FURTHUR REQUEST FROM MANAGEMENT.
5) QUARTERLY TRIMMING OF PERIMETER FICUS HEDGE ALONG THE SCHOOL TO THE DEAD
END.
FERTILIZATION:
AS WITH ANY LIVING ORGAMSM, A BALfiNCED DIET AND MINIMAL STRESS PROVIDES AN
ENVIRONMENT FOR BEST GROWTH, DISEASE RESISTANT AND BEAUTY OF FORM AND COLOR.
SEVERAL IMPORTANT FACTORS MUST WORK TOGETHER TO PROVIDE THE DESIRED RESULT.
FOR EXAMPLE, SKIMPING ON NUTRITION MAY NOT BE IMMEDIATELY EVIDENT 8UT IN TIME
WEEDS BEGIN TO ENCROACH, DISEASE RESISTANCE WEAKENS AND IN MANY CASES, SUCH AS
WITH PALMS AND PWES, A CHEMICAL SCENT IS GIVEN OFF THAT QECOMES AN IMMEDIATE
INVITATION TO WSECT ATTACK. THEREFORE A COOPERATIVE EFFORT IS REQUIRED FOR BEST
RESULTS. ONLY ACID BASED, GRANULAR FERTILIZER IS USED WITH A CONTROLLED RELEASE
NITROGEN SOURCE.

WHITEFLY, ROYAL PALM BUG AND NEW AND/OR EXOTIC SPECIES ARE NOT INCLUDED. RELYING

NEXTERA, LLC IS A FULLY LICENSED AND INSURED CONTRACTOR. WE WILL UPON EXECUTION
OF THIS AGREEMENT, SEND WITHIN SEVEN (7) DAYs, A CERTIRCATE OF WSURANCE NAMING
SIENA OAKS HOMEOWNERS ASSOCIATION AS AN ADDITIONAL INSURED ON OUR POLICY. TO
INITIATE THIS SERVICE/AGREEMENT SIMPLY SIGN AND DATE IN THE SPACE PROVIDED BELOW
AND RETURN ONE (1 � ORIGINAL COPY SO WE MAY RETAIN IT FOR OUR FILES.
A 1�k% INTEREST RATE ON PAST DUE ACCOUNTS SHALL APPLY & ACCUMULATE BEGINNING
WITH FIRST BUSINESS DAY DEEMED LATE ACCORDING TO THE TERMS DESCRIBED BELOW...
ANNUAL PERCENTAGE RATE IS 18%. ANY COSTS INCURRED AS A RESULT OF NON- PAYMENT,

NEED TO RENEW THIS CONTRACT AT THE EXPIRATION (ONE YEAR FROM COMMENCEMENT
DATE�. HOWEVER, IF AT ANY TIME EITHER PARTY WISHES TO TERMINATE SERVICE THEY MAY
DO SO BY SENDING THE OTHER A THIRTY (30) DAY CERTIFIED WRITTEN NOTICE SPECIFYING
REASON, OR NOT FOR TERMINATION. CONTRACTS TERMINATED AND FINfiL BILLING WILL BE

BE NO INCREASE REQUEST IN OUR FIRST OR SECOND YEAR. HOWEVER WE WILL REOUEST A

DESCRIBED IN THE AFOREMENTIONED PARAGRAPH ABOVE.
NEXTERA WILL UPON EACH SERVICE PROVIDE AN ACCOUNT MANAGER TO
OVERSEE AND MANAGE THE CREWS PERFORMANCE, AS WELL AS TO ADDRESS
ANY HOMEOWNER NEEDS. YOUR ACCOUNT MANAGER ASSIGNED TO SIENA
OAKS WILL BE MR. LARRY KATZ.
UPON REVIEW OF THE COMMUNIIY WE TOOK NOTE TO SEVERAL ISSUES THAT
REQUIRE ATTENTION, AS WELL AS POTENTIAL OPTIONS OFFERED TO THE
COMMUNITY. THERE ARE MANY HOMES THAT HAVE INOPERABLE IRRIGATION
SYSTEMS, OR ARE OPERATING AT LESS THAN DESIRABLE LEVELS TO PROMOTE
LUSH, AND HEALTHY TURF. THE CONDITION OF MOST OF THE LAWNS, AS
WELL AS ORNAMENTALS ARE IN STEADY DECLINE AS THEYARE INFESTED WITH
FUNGI , WHITE FLY, SPIRAL WHITE FLY FOR EXAMPLE. FERTILIZATION, WITH
OPERABLE IRRIGATION SYSTEMS SHOULD HAVE PREVENTED MANY OF THESE
ISSUES THAT HAVE BECOME APPARENT.

THE HOMES. THIS VVILL REQUIRE ADVANCED SCHEDULING TO BE ABLE TO

ASK THAT MATERIALS BE BILLED SEPARATE AT COST (IE, HEADS< NOZZLES<
MINOR LATERAL BREAKS) THESE REPAIRS WOULD BE PERFORMED AT THE TIME
OF THIS WET TEST. ALONG WITH THE WEf TEST WiLL BE A REPORT FOR EACH
OF THE HOMES MADE AVAII..ABLE. THIS REPORT WILL DEfAIL ANY WORK THAT
WAS PERFORMED FROM ZONE TO ZONE, AS WELL AS DESCRIBE ANY
OUTSTANDING ISSUES NEEDING FURTHER APPROVAL.
FURTHERMORE IT IS OUR INTENTION TO CONTINUE THIS SERVICE ON A BI-
MONTHLY BASIS WITH HOMEOWNERS THAT REQUEST TO BE INVOLVED IN
THIS PLAN. THEY WILL HAVE THE CHOICE TO ELECT TO BE APART OF THIS PRO
ACTIVE PROGRAM, OR SIMPLY DECL►NE.
THE MONTHLY COSTS ARE GOING TO BE APPROXIMATELY $15-$20.00 PER
MONTH FOR EACH HOME. THESE FIGURES ARE AT THE MOMENT AN
APPROXIMATE COST. FOR THOSE THAT ELECT TO BE A PART OF THIS
PROGRAM, NEXI DOES HAVE THE ABILIIY TO PROVIDE THEM
