Subject : Resolution 94-15 Supplemental bid sheets
Date : February 1, 2016
The Village of Tequesta received 4 bids in October 2015 for (1 � the monthly maintenance of
the Village's right of ways and (2) basic service rates for extra work performed outside the
specifications of the monthly maintenance. Terracon Services Inc. won the bid.
Council approved the monthly mainfenance portion of the bid on November 12, 2015 with
Resolution #94-15.
We are asking fhat Council accept and approve the additional service rates submitted by
Terracon.
Any additional services will require a quote matching pricing in Terracon's bid sheet and will
be paid when services are rendered.

The tota� is an annuai cost on this sheet is an annual price (12 months}.
Items 14 (Uover Ditch) and 17 (Russell St Pondi can be added fnr $18,000. per year. See attached
pricing and schedule
A compiete check and adjustment of ihe irrigation system will take place once per month. All clocks
w�ili be checked and set at this tune. All additional work includin breaks and raisin ���

This contract may be canceled by either party with a thirty day written nodce Terracon Services, Inc.
reserves the right to ce�se services without notice if the Owner's account becomes thirty (30) days past OAO
due.
All work will be performed with supervision. A supervisor will be available for walk around
inspections as deemed necessar� by the customer. Any repairs or extra work done nutside the ��
specifications of this agreement wil! be dune at an additional charge
Terracon Services, Inc. as a business is licensed and quahfied to spray lawns and ornamentats in the
State of Florida Terracon Serviees, Inc. is licensed by Palm Beach County to perform irrigation work ��
Terracon Services, Inc. agrees to maintain general liability and workers compensation insurance.

ADDENDUM
Public Records: In accordance with Sec. 119.0701, Florida Stata�tes, the Contractor
must keep and maintain this Agreement and any other records associated therewith and that are
associated with the performance of the work described in the Scope of Services. Upon request,
the Contractor must provide the public with access to such records in accordance with access and
cost requirements of Chapter 119, Florida Statc�tes. Further, the Contractor shall ensure that any
exempt or confidential records associated with this Agreement or associated with the
performance of the work described in the Scope of Services are not disclosed except as
authorized by law. Finally, the Contractor shall retain the records described in this paragraph
throughout the performance of the work described in the Scope of Services, and at the conclusion
of said work, transfer to the Village, at no cost to the Village, all such records in the possession
of the Contractor and destroy any duplicates thereof. Records that are stored electronically must
be transferred to the Village in a format that is compatible with the Village's information
technology systems.
