These stretchers use battery power to raise and lower the stretcher for patients weighing up to 500 pounds. The stretchers cost $39,154.00 each. The total grant application is $78,308.00. This matching grant is a 75 % -25% grant in which the Village would have to appropriate funds to pay 25% or a total of $19,577.00 and the State would pay $58,731.00. Back injuries are one of the most common injuries in the Fire Service and can cost both the Firefighter and the Village considerable time, money and pain. These stretchers are intended to reduce the risk of back injury and thereby protect both the employee and the Village and it is respectfully requested this application be approved and the 25% be appropriated by the Village. Justification Summary A) Problem Description —We are living in a world of increasing patient weight and increasing technological patient care adjuncts, most of which are carried on the ambulance cot. The end result is that the amount of weight that we currently carry on the ambulance cot is more than ever before. It has been well documented that repeated lifting of patients from the lowered cot position creates long -term back problems in some EMS workers. Additionally, acute back injuries happen with regularity and can, in some cases, be career ending. B) Our organization runs two ALS Ambulances, both of which are equipped the Ferno Model 93H Pro -Flex ambulance cot. In its day this cot was the cot of choice but technology has created alternatives that have made this cot less than optimal. Additionally, these cots are 11 years old and in need of replacement. C) We are proposing to replace these cots with the Stryker Power Pro XT Ambulance Cot, including the Power -LOAD Fastening System for mounting the cot in the ambulance. We have received demonstrations from several manufacturers and the recommendation of our committee, along with the general consensus of our staff, is to purchase the Stryker Power Pro XT. D) This purchase was not able to be budgeted for this year and we don't expect it to survive the budgeting process next year either. It is most likely that if this project is not funded we will not be able to purchase new power cots in the foreseeable future. E) The geographic area that will be covered is the Village of Tequesta which covers approximately 2 square miles and surrounding areas when Tequesta Fire Rescue is requested for mutual aid. Surrounding areas include the Jupiter Inlet Colony, Palm Beach County and Martin County. Tequesta Fire - Rescue is included in a "Closest Unit Response" Automatic Aid agreement. The result of this is that we routinely respond outside of our Village limits. These ambulance cots will be used to provide care to over a thousand patients a year and reduce the potential of back injury to 21 Paramedics and EMT's. F) The proposed times frames that we feel confident in meeting are as follows: a. Month one after receiving program support —Order Stryker Ambulance Cots. b. Month two — Receive the cots. c. Month three — Train on operation of the cots and have the hardware installed into the two Ambulances. d. Month four — Place cots into operation. G) Data Sources a. Farifield County EMS Study on Employee Absenteeism (http: / /ems.stryker.com) H) I hereby attest that Tequesta Fire - Rescue has never used State grant monies, nor any other grant monies, for the purchase of ambulance cots. This project does not duplicate any previous grant project from any source, including the State of Florida. 13. Outcome for Other Projects A) We have been using the Ferno Model 93H Ambulance Cot for patient movement. This requires two or four people to pick the cot up from a knee - height, which puts strain on the back muscles. Depending on the patient size and equipment placed on the stretcher this load can be significant. During calendar year 2015 we saw eight missed days of work due to back pain /injury. B) If our project is funded we anticipate the placement of power stretchers on both of our Ambulances as quickly as possible, but certainly within the four month anticipated "Work Activity and time frames" worksheet. All of our employees will be trained on the proper use of the power stretcher and the vehicles will be equipped with the Power -LOAD mounts. This will ersult in a dramtic reduction in the use of back muscles to safely load patients into and out of the Ambulances. C) In the calendar year 2015 employees of Tequesta Fire - Rescue missed 8 days of work due to back pain or back injury. None of these injuries resulted in permanent injury, however each of them had the potential to. The lifting of patients during a 9 -1 -1 call been the primary reason for back injuries since the inception of this department in 1993. We anticipate a reduction of 66% of missed days due to back pain /injury based on the studies and articles that we referenced in 10) G. D) Employee injury information was obtained from the human resources department of the Fire Department. Our anticipated results are our best estimates based on the studies and articles that we reviewed, some of which are listed in question 10 -G). E) To be honest, our Departments five -year plan does not address this. However, I think that an attempt to reduce employee risk, injury and sick leave would be part of anyone's plan for their department. Tequesta Fire - Rescue has always been employee oriented and anything that we can do to reduce the risk of injury to our employees is something that we want to do. The budget of this small department cannot support this project and we're hoping that you will see fit to support this. ADDENDUM Public Records: In accordance with Sec. 119.0701, Florida Statutes, the Contractor must keep and maintain this Agreement and any other records associated therewith and that are associated with the performance of the work described in the Scope of Services. Upon request, the Contractor must provide the public with access to such records in accordance with access and cost requirements of Chapter 119, Florida Statutes. Further, the Contractor shall ensure that any exempt or confidential records associated with this Agreement or associated with the performance of the work described in the Scope of Services are not disclosed except as authorized by law. Finally, the Contractor shall retain the records described in this paragraph throughout the performance of the work described in the Scope of Services, and at the conclusion of said work, transfer to the Village, at no cost to the Village, all such records in the possession of the Contractor and destroy any duplicates thereof. Records that are stored electronically must be transferred to the Village in a format that is compatible with the Village's information technology systems.
