SICK LEAVE BUY BACK APPROVAL: MICHAEL COUZZO, MANAGER ( Effective '_ ��.:: - October 1 of each fiscal year, TEQLJESTA, FLORIDA any eligible emptoyee who has been continuously employed for at least twelve (12) months and who has an accrued sick leave balance of 480 hours, and who has taken no more than forty (40) hours of sick leave during the twelve ( I 2) month period immediately preceding September 30 of each fiscal year, may sunender 40 hours for cash payment at one hundred percent (100°lo) of their current hourly rate. Any additional eligible hours may be redeemed at fifty percent (50%) of Cheir current hourly rate. However, employees must have at least 360 hours of sick leave remaining after surrendering sick leave for cash payment, to be eligible to buy back sick leave I under these provisions. Pavment will be made on the first payroll run in December of each vear. VIII. TERMINATION: Employees who resign with the proper two-week notice, who have not been terminated for cause and who have been an employee for at least one year are reimbursed for a portion of sick leave accrued. Payment for any sick leave shall be provided to those full-time and part-time who have successfully completed their probationary period. Temporary employees and Interim Firefighters are not eligible for sick leave payment. The amount oP reimbursement shall be a percentage of any accrued sick leave, up to the maximum accrual amount. The percentage is based on whole completed years of 3
