Dear Mr. Pastuch:
I am in receipt of your several e-mails to the Village of Tequesta, including public
records requests, regarding the above referenced matter. I have consulted with various
Village personnel involved with the sidewalk replacement project and the following is the
Village's position on the matter and response to your correspondences on the topic:
In September, 2017, with the approval and adoption of the 2017/2018 budget via
Resolution 45-17, the Village Council authorized the expenditure of funds for the
Sidewalk Replacement Project.
The Sidewalk Replacement Project was competitively bid in September, 2017 via
RFP No. PW03-17. Pursuant to the RFP process, R&D Paving was determined to be
tne proposer that best served the Vilfage's interests and needs.
On September 20, 2017 the Village awarded the contract for performance of the
Sidewalk Replacement Project to R�D Paving, the contract was approved by the Village
Council and on October 12, 2017 was executed by the Mayor.
Between the months of October, 2017 and April, 2018, the Sidewalk Replace-
ment Proje�t Work was performed by R&D Paving. As various aspects of the project
were peRnitted through the Building Department, they w ere also inspected and deter-
mined to be in compliance with the applicable requirements of the Florida Building
Code.

Village of Tequesta Sidewalk Replacement Projecf;Response tn Complaints Raised
As the documentation which establishes the above facts reveals, the Sidewalk
Replacement Project was properly budgeted and was approved to be implemented by
the Village Council; the project was competitively bid and the contractor was selected in
accordance with Village procurement requirements; the work was performed in accord-
ance with industry best practices; upon permitting and inspection by licensed inspec-
tors, the work was determined to meet applicable Florida Building Code requirements.
Although we have reviewed your numerous complaints, it is the Village's position that
they are unfounded, and that the sidewalk replacement project is a success. There is
no question that the new concrete sidewalks are a vast improvement over the old
asphalt sidewalks and wil{ be an asset to the Village for many years to come. Be
advised that the Village of Tequesta, having addressed your complaints and having
responded to your public records requests, now considers this matter closed. You
should not expect any further response from the Village of Tequesta related to the
sidevralk replacernent projec:.

Engineer. Large boulders and other obstructions shall be removed to a minimum depth
of 6-inches bebw the finished subgrade elevation,and the space shall be backfilled with
sand, base course material or other material or suitable material which shall be
thoroughly compacted by rolling or tamping.
• The subgrade shall be accurately trimmed to the required elevation with a Y-inch
tolerance. High areas shall be trimmed to proper elevation. Low areas may be filled with
suitable material and compacted to the specified density or filled with concrete
integrally with the placing of the pavement.
• (2)Setting Forms:The forms shall be accurately set to line and grade and such that they
rest firmly,throughout their entire length upon the compacted subgrade surface. Forms
shall be joined neatly and tightty and braced to resist the pressure of the concrete and
the finished operations. The alignment and grade of all forms shall be approved before
and immediately prior to the placing of concrete.
• (3) Slipforming: The slipforming method will be allowed, provided that an acceptable
finished product,true to line,grade,and cross section is consistently produced.
• (4) Mixing Concrete: Concrete shall be mixed in accordance with the requirement of
Section 03000.
c. Installation
(1) Placing concrete:
• The concrete shall be distributed on the subgrade to such depth that, when it is
consolidated and finished,the thickness required by the drawings will be obtained at all
points and the surface will at no point be below the grade specified for the finished
surface.The concrete shall be deposited on the subgrade in a manner which will require
as little �ehandling as possible. Placing of the concrete shall be continuous between
transverse joints,without the use of intermediate bulkheads.
• Reinforcement shall be placed and maintained at this location during the placing and
finishing operation.
• Concrete shall be thoroughly consolidated against and along the faces of all forms by
means of vibrators. Vibrators shall not be permitted to come in contact with the
subgrade or a side form. �bration at any one location shall not continue so long as to
produce puddling or the accumulation of excessive grout on the surface. In no case shall
the vibrator be operated longer than 15 seconds in any one location.
(2) Striking-off, Consolidating and Finishing Concrete: Immediately after the placing, the concrete
shall be stuck off, consolidated and finished, to produce a finished product conforming to the
existing area and or required by the drawings,Specifications and Instructions.
(3) Straightening and Surface Corrections: After floating has been completed and the excess water
removed, but while the concrete is still in a plastic state, the surFace of the concrete shall be
tested for trueness with an accurate 10-foot st�aightedge.The straightedge shall be furnished by
the Contractor.The straightedge shall be held in successive positions parallel to the walk center
line,in contact with the surface,and the whole area tested from the side of the slab to the other
as necessary.The advance along the walk shall be in successive stages of not more than one-half
the length of the straightedge. Any depressions shall be immediately filled with freshly mixed

You have requested information regarding the above-referenced Permits for the Sidewalk
Rehabilitation and Construction project located on the south sideof Tequesta Drive between Seabrook
Road and Country Club Drive (the Project). The above-referenced permits(the
Permits) have been issued and

finalized as they are in accordance with the contractual requirements as well as the Florida Building
Code.
The contractor for the Project, R&D Paving LLC, was chosen by a Request for Proposal (RFP)
committee review. A signed contract was provided as partof the RFP review which detailed the Project
requirements, including the width and depth of the paving concrete.
On April l8, 2018, a final inspection ofthe Project was performed by me in my capacity as
Building O�cial for the Village of Tequesta.
I inspected 15 different areas of the Project to verify the
concrete depth, as documented in the attached photographs. I verified that the sidewalk is at least4
inches thick and the concrete in the driveways are at least 6 inches thick. Accordingly,the
Permits have been issued for the Project.
Attached,for your review, is a copy of the Permits and supporting documentation,the
inspection photographs and the contract.

ADDITIONAL EXPENDITURES IN THE PUBLIC WORKS DEPARTMENT FOR SIDEWALK REPAIRS.
We are currently repairing the sidewalk on the South side of Tequesta Drive from Seabrook Road to Riverside
Drive.tt would be beneficial and desirous to continue the repairs on the South Side of Tequesta Drive from Point
Drive to Country Club Orive.
