SUBJECT: AMBULANCE PURCHASE FOR EM R NCY MEDICAL TRANSPORT SYSTEM.

At the August 15, 2006, budget workshop meeting the purchase of
a new ambulance was discussed and budgeted. (Attached are the
minutes of that meeting - Exhibit 1). The biggest discussion was
when to purchase this vehicle.
In the minutes I stated that I didn't think I could put off this
purchase for another three years, but I could put it out to bid
for late 2007. If this purchase request is approved, the vehicle
would not be delivered until 300 days after the Purchase Order
is received by the manufacturer.
The vehicle being replaced is a 1997 E-450 Ford Wheeled Coach
Ambulance. The vehicle has low mileage, approximately 29,000
miles, but mileage on emergency vehicles is not a good indicator
for vehicle replacement; these vehicles spend a lot of time
idling. As with any vehicle there becomes a point when it' s not
dependable because of age and wear and tear. This vehicle had to
be towed to a repair shop twice last year and we had to borrow
an ambulance from the County for a week last year.
Since January 1, 2006, we used this back-up ambulance 24 times
to transport patients. This vehicle is used when the primary
unit is out for repairs or service, when we have simultaneous
calls, and during special events. Although we do not have an
accurate account of time for days out of service, we did note
being down to one ambulance for 40 days due to repairs and/or
maintenance during this same time frame.
I feel it is imperative that we have two reliable ambulances in
Tequesta to effectively provide pre-hospital care for our
residents. Currently we have the resources available from Palm
Beach County Station #11, but that contract is set to expire in
July 2008 and Chief Brice has told me it's his intention to

close that station. If this request is approved the replacement
ambulance will be in service before the expiration of that
lease.
In an attempt to reduce costs we went back to the original
manufacturer of the vehicle to see about just changing out the
chassis. Attached is letter from Wheeled Coach dated May 31,
2007, that explains why this cannot be done (Exhibit 2).
The three bids for this vehicle are as follows:
1. Wheeled Coach $195,400.00 (Exhibit 3A)
2. American LaFrance $193,090.74 (Exhibit 3B)
3. Medtec $199,004.00 (Exhibit 3C)
Medtec has offered a 10,000.00 trade-in which would reduce their
price to 189,004.00, but I feel we can sell this vehicle out
right for double that amount.
Therefore, it's my recommendation to move forward with this
purchase from American LaFrance for $193,090.74.
Attached is a memorandum from our Finance Director outlining the
means available to purchase or finance this vehicle. The
decision on financing does not need to be finalized until
acceptance of the vehicle, which is estimated to be in May or
June of 2008.
If you see fit, please place this on the Village Council Agenda
for discussion and authorization for this purchase.

The Fire Department is proposing the purchase of a new ambulance for EMS Transport and has
requested I prepare a financing plan for this item. Per my discussion with Chief Weinand I
understand the following:
1. The estimated cost of the ambulance is $193,090.00
2. Industry-wide the average life of the ambulance is 7 years
3. The Fire Department would like to order the ambulance by August 1, 2007
4. The ambulance takes 300 days (approx. 10 months) to build and the Fire Department expects
to take delivery of the ambulance June 1, 2008
Based upon the above information and the current economic environment, I would recommend that
the Village Finance the ambulance over a term of 7 years with monthly payments paid in arrears.
Based upon this recommendation I contacted Bank of America Public Capital Corp., pursuant to the
Master Lease/Purchase Program in which the Village participates and received the following
information regarding financing of the new ambulance;
1. Current interest rates are 4.78% or 94% of the 7-year Treasury. Since they use the
Bloomberg rates, they are unable to tell us what the rates will be over the next 10 months.
The rates when quoted are good for 30 days.
2. Lease term: Eighty-four (84) months paid in arrears
3. Monthly payment (P&1) under these terms = $2,709.20
4. First payment due one month after funding.
5. Estimated 2008 debt service cost (4 months) _ $10,836.80
6. Future budgets: annual cost (12 months) = 32,510.40

A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF
TEQUESTA, PALM BEACH COUNTY, FLORIDA, APPROVING THE PURCHASE
OF ONE (1) RESCUE VEHICLE (AMBULANCE), IN THE AMOUNT OF
$193,090.74, FROM AMERICAN LA FRANCE, LLC, OF SANFORD,
FLORIDA, THROUGH A PIGGY BACK PURCHASE FROM ST. JOHNS COUNTY
BID #07-67 FOR AMBULANCES, AND AUTHORIZING THE VILLAGE MANAGER
TO EXECUTE THE APPLICABLE CONTRACTS ON BEHALF OF THE VILLAGE.
NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF THE VILLAGE OF
TEQUESTA, PALM BEACH COUNTY, FLORIDA, AS FOLLOWS:
Section 1. The purchase of one (1) custom rescue vehicle (ambulance) awarded
to American LaFrance, LLC, of Sanford, Florida, in the amount of
$193,090.74, in accordance to American LaFrance, LLC Ambulance
specifications, attached hereto as Exhibit ~~3B" and incorporated by
reference as part of this Resolution is hereby approved and the Village
Manager is authorized to execute the applicable Contract on behalf of the
Village.

Vice Mayor Paterno asked if the Village was going to buy another ambulance. Chief
Weinand stated it was a replacement for the 1997 model ambulance. Village Manager
Couzzo asked Chief Weinand if the 1997 model was in service for 10 years. Chief
Weinand stated yes. Village Manager Couzzo mentioned the 2005 model ambulance was
not received until 2006. Chief Weinand stated the Fire Department bought them in
January of 1997. Vice Mayor Paterno stated he could not find anything on the vehicles.
He asked Chief Weinand how much it would be. Chief Weinand stated he budgeted
$165,00 to $170,000. Vice Mayor Paterno asked how the Village would pay for it. Chief
Weinand stated it would be under afive-year lease. Vice Mayor Paterno asked since they
were changing the fire rescue, the one changed would be a back up. Vice Mayor Paterno
suggested postponing buying it a few more years so that they don't wait long periods of
time for new EMS vehicles and equipment. Council Member Genco stated she wanted to
see the schedule to see what the Village was buying, what s the debt and what are the
projected increases. She stated one of the things she asked for what the l0year projection.
Vice Mayor Paterno stated he would like to see the Village getting a truck every five
years, so that one moves to the front and the other becomes secondary. Chief Weinand
stated he did not think he could stretch it 3 years but another year was possible. He
explained the Fire Department could put the bid out in the middle of 2007 and have it
delivered in October. Vice Mayor Paterno asked if the Village pays $160,000 how much
would they pay in total. Michelle Gload of the Finance Department stated the annual
payment for the ambulance would be $24,300 including interest and principal. She
referred Council Members to "Debt Service." Ms. Gload stated that was included in the
$20,000; it would be $11,000 principal and $13,200 interest. Ms. Gload stated the total
would be $78,000. Council member Genco referred to page 41 and asked about the
interest lease decrease. Ms. Gload explained the Village would pay more in principal and
less in interest each year. Vice Mayor Paterno asked if the last ambulance was 10 years.
Chief Weinand stated it was 5 years for the last one. Vice Mayor Paterno felt $78,000 on
$165,000 is a lot of money. Council member genco stated she thought the Village got a
good rate. Vice Mayor Paterno pointed out Ms. Gload projected 8.2% but they won't
know the exact amount yet. The one that is there has the lease, but they haven't done
the 10 year lease on the new one. Council Member Genco commented she would like to
see, on a flat level, exactly what the hard assets they were paying far. She expressed she
would like to know how it affected the Village's budget in the years to come versus what
the Village's revenues were. Council Member Genco stated everything led back to 401.
She felt if it was not for 40i, the Village would not be in the best shape and commented
the 401 was brought in revenues the Village could transfer over. Vice Mayor Paterno felt
the budget was organized in such a way that Council had to interpret it and it was
convoluted and difficult to follow. Council Member Genco expressed she sees a
significant improvement in the way staff dealt with its budget but she still had issues
related to the overall Village Management. She commented she does not want any
Village Employees to be offended.
Vice Mayor Paterno commented he would like to see the EMS trucks ordered with a
closer time between the replacement period so the Village always had a new truck and a
second one as backup for better service to Village residents. Council Member Genco

suggested once a quota was built into the expenses, it would not matter what the
replacement time was. Vice Mayor Paterno stated the money was not an issue and it had
to do with having newer trucks to service Village residents. Chief Weinand stated the
newest EMS truck was a 2005 model. He explained the original attempt was to try
staggering the replacement period of the EMS trucks because two trucks were bought in
1997, and at that time, he was under the impression that model would have been 7 years.
He stated he tried to push it to ten years and commented he probably bought the 2005 too
late and so he was trying to stagger it to get trucks every 3 to 5 years. Council Member
Genco commented if the Village knows it would spend $500 for a certain vehicle every
year then the replacement cycle was not as important as long as the Village knew what
the budget was. V ice Mayor Paterno commented that he understood that aspect and
commented he was more concerned about keeping a newer EMS vehicle on the road.
Planning
Development Report on the budget stated what the department had accomplished and
their future goals. She stated the budget she was presenting would be a balanced budget
and explained the net would be $17, 870 if everything goes as projected. She explained
she was trying to make it balanced so the Building Department could cover itself in its
operating cost. She stated she asked for two new employees in the middle of the year. She
pointed out one position was a Building Inspector and the other was a planner. She stated
the department finally hired a Building Inspector who had been working for the Building
Department for two weeks. She stated he was hired with a salary of $63,000 and he has
replaced an exorbitant outside agency, which cost more than $108,000. She commented it
has cut the expense to the Village in half and has tripled the service to Village residents.
She stated if the department runs a balanced budget as anticipated and with her already
taking on the role of the planner, they have decided to drop the planner position from the
budget completely. She explained she talked to Village Manager Couzzo and it was
decided to leave their options open, and in June, if money was flowing, the budget stayed
in balance, and things were going well economically, they would consider if a planner is
necessary. Village Manager Couzzo stated what Ms. Harding appropriated would not be
spent through out the calendar year for the position of the planner that Council previously
approved. He explained next year, instead of appropriating money for an entire position,
money has only been appropriated for b months. Council Member Genco stated the
$38,200 under Personnel Services would be appropriated if the Village did not hire
anyone. Village Manager Cauzzo stated it was correct and stated the department would
still have the operating expenses. Vice Mayor Paterno suggested cutting the position out
completely and later, if it was decided a planner was needed, the money would be re-
appropriated. Council Member Genco suggested Ms. Harding go back to Council if she
needed a planner. Ms. Harding commented that was fine as long as that door was open
that.
Building and Zoning
Ms. Harding explained the section is for the building, liscensing and zoning, which
means with the building permit issue there is a zoning approval on that permit. She
explained all the contractors that work for the Village and all the busnesses in town are
licensed and that encompasses the building and zoning g area. She explained they have

Thank you for contacting us with regard to your Wheeled Coach Type 3 ambulance,
Work Order #963320, VIN: 1FDLE40F7VHA15576. This vehicle was evaluated on
May 22, 2007. On that date the odometer registered 28,270 miles. The unit appears to
have been well cared for and is in very good condition. With regard to trade-in's our
company policy does not allow the offer of a trade on a vehicle that is more than ten
model years old. Ford is currently building 2008 model year ambulance chassis. The
wholesale trade value of this unit would be around $12,000.00 to $15,000.00 if we were
able to make an offer. If the department sold the vehicle outright it could bring from
$23,000 to $28,000.00 at full retail due to its condition and low mileage. However, it is
unlikely that these values would be received in a municipal auction. If desired, we can
provide assistance in ways to advertise this unit nationally and/or other potential ways
that this unit can benefit the City of Tequesta and the Fire-Rescue Department.
With regard to the possibility of remounting there are a number of things to consider.
1) This module was designed to mount on a Ford Ambulance Prep E-Series
Cutaway Chassis with a 158" wheel base. This chassis is not currently being
manufactured by Ford. Therefore an exact replacement chassis is not
available at this time. The only current production chassis currently available
with sufficient GV W to carry this module in an ambulance configuration
would be a medium duty chassis.
2) There is no medium duty chassis that is a direct replacement for the Ford
Ambulance Prep E-Series Cutaway chassis with 158" wheel base.
3) The frame rail height above the ground of potential medium duty chassis is
higher than that of the Ford Ambulance Prep E-Series Cutaway Chassis. This
would result in the appearance that the ambulance was much higher up in the
air than it should be. The body sides would simply be too short.

4) Of more importance than appearance is the floor height of the module. To
meet the KKK-A-1822E ambulance standard the floor height cannot exceed
33". Remounting this module onto a medium duty chassis does not alter this
requirement. The floor would have to be at or below 33" after the chassis was
in the air dump position. If the floor is too high it will not only fail to meet
the standard but will make it difficult to load a cot with a patient on it. This
will compromise patient safety and may cause back injury to the crew.
5) If the module were to be remounted onto a medium duty chassis the front of
the module would have to be restructured. As built, the front of the module
was designed to be bolted directly to the back of the Ford cab. Additionally,
the mounting sill plates that secure the module to the chassis would have to be
relocated. To perform these tasks correctly the entire interior would have to
be removed. If the structural work is not properly performed the safety of the
vehicle would be compromised. Lastly, this work would have to be done by
Wheeled Coach or a Wheeled Coach Service Center authorized to do such
work to ensure safety and maintain the structural warranty of the module.

Chassis proposed, and a specification for the emergency medical units. The
production of these units will be completed in approximately 300 days after

Said apparatus is to be shipped in accordance with the specifications hereto attached, delays due to strikes, war or
international conflict, failures to obtain chassis, materials, or other causes beyond our control not preventing, within about 180 - 220
days after receipt of signed written order at our office at Goshen, Indiana, and receipt of chassis, to be delivered to you at Tequesta,
FL.
The specifications herein contained shall form a part of the final contract, and are subject to changes desired by the
purchaser, provided such alterations are interlined prior to the acceptance by the company of the order to purchase, and provided such
alterations do not materially affect the cost of the construction of the apparatus.
The proposal for emergency medical apparatus conforms to all Federal Department of Transportation (DOT) rules and
regulations in effect at the time of bid, and all (KKK-A-1822E) Federal Specification for Star of Life Ambulance and (AMD)
Ambulance Manufactures Division Guidelines for Automotive Emergency Medical Apparatus as published at the time of bid, except
as modified by customer specifications. Any increased cost incurred by first party because of future changes in or said DOT, KKK or
AMD standards will be passed along to the customer as an addition to the price set forth above.
Unless accepted within 30 days from date, the right is reserved to withdraw this proposal.
